SUBJECT: NOTICE OF V10LAT10N

(NRC INSPECTION REPORT N05. 50-327/88-46 AND 50-328/88-46)

This refers to the special Nuclear Regulatory Cormiission (NRC) heatup

shift coverage inspection conducted by Ken Jenison from October 5, 1988

througt October 11, 1988. The inspection included a review of the activities

associated with the heatup of Sequoyah Unit 1. At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. These areas

included extended control room observation and operational safety verification.

Within these areas, the inspection consisted of selective examinations of pro-

cedures and representative records, interviews with personnel, and observation

of activities in progress.

The inspection findings indicate that certain activities appeared to violate

NRC requiretrents. The identified violation involved a failure to implement a

fire watch for non-functional fire barriers per the action statement of

Technical Specification 3.7.12. During this inspection and your subsequent

investigation, numerous additional deficiencies were noted in the fire watch

routes. It is expected that your response to this violation should include

resolution and corrective actions for all the identified deficiencies. The

violation, references to pertinent requirements, and elements to be included in

your response are described in the enclosed Notice of Violation.

In accordance with Section 2.790 of the NRC's "Rules of Practice " Part 2,

Title 10, Code of Federal Regulations, a copy of this letter, its enclo:ures,

and any reply will be placed in the NRC's Public Document Room.

The responses directed by this letter and its enclosures are not subjected to

the clearance procedures of the Office of Management and Budget as required by

the Pape work Reduction Act of 1980, PL 96-511.
