This refers to the routine Nuclear Regulatory Commission (NRC) resident

inspection conducted by Ken Jenison from September 13 - October 5,th those

1988.

At

the conclusion of the inspection

the findings were discussed wi

members of your staff identified i,n the enclosed inspection report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of selective examinations of procedures

l

and representative records, interviews with personnel, and observation of

activities in progress.

The inspection findings indicate that certain activities appeared to violate

!

NRC requirements.

The violation identified involved a failure to follow

i

precaution instructions associated with the withdrawal of the incore probe from

t

its tube enclosure. The violation

references to pertinent requirements, and

elements to be included in your re,sponse are described in the enclosed Notice

i

of Violation.

One unresolved item was identified concerning the adecuacy of fire watch

inspections.

This matter will be pursued during future inspections.

In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2

Title 10, Code of Federal Regulations, a coay of this letter, its enclosures,

and any reply will be placed in the NRC's Pu)lic Document Room.

the clearance procedures of the Office of Management and Budget as required by
