This refers to the routine Nuclear Regulatory Commission (NRC) resident  :

members of your staff identified i,n the enclosed inspection report.

Areas examined during the inspection are identified in the report. Within 1

these areas, the inspection consisted of selective examinations of procedures l

and representative records, interviews with personnel, and observation of

activities in progress.

The inspection findings indicate that certain activities appeared to violate  !

NRC requirements. The violation identified involved a failure to follow i

precaution instructions associated with the withdrawal of the incore probe from t

its tube enclosure. The violation references to pertinent requirements, and  ;

elements to be included in your re,sponse are described in the enclosed Notice i

of Violation.

One unresolved item was identified concerning the adecuacy of fire watch ,

inspections. This matter will be pursued during future inspections. (

In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2  !

Title 10, Code of Federal Regulations, a coay of this letter, its enclosures,  ;

and any reply will be placed in the NRC's Pu)lic Document Room. '

The responses directed by this letter and its enclosures are not subjected to

the clearance procedures of the Office of Management and Budget as required by  :

the Paperwork Reduction Act of 1980, Pi 96-511. i
