Work request, WR B296449 contained a caution state-ment to be adhered to while performing a osit on calibration on the Unit 2 f

The caution speci fed that the storage reel be t
locked when using the drive unit to drive the detector to the storage t
location.
i' Contrary to the above, on September 9 1988, licensee personnel failed i
to r.omply with the precautions listed in WRB296449, resulting in an inadvertent withdrawal of the incore probe from its tube enclosure.
