Dear Mr. White:

SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT N05. 50-327/88-40 AND 50-329/88-40)

This refers to the special Nuclear Regulatory Commission (NRC) heatup shif t

coverage inspection conducted by Ken Jenison from September 25 - October 4,

1988. The inspection include'.' a res lew of the activities associated with the

heatup of Sequoyah. At the conclusion of the inspection, the findings were

discussed with those members of your staff identified in the enclosed inspec-

tion report.

Areas examined during the inspection are identified in the report. These areas

included extended control room observation and operational safety verification.

Within these areas, the inspection consisted of selective examinations of pro-

cedures and representative records, interviews with personnel, and observition

of activities in progress.

The inspection findings indicate that certain activities appeared to violate

NRC requirements. The identified violation involved a failure to meet the

requirements specified for conducting test activities and resulted in one

reactor trip breaker opening. The violation, references to pertinent require-

ments, and elements to be included in your response are described in the

enclosed Notice of Violation.

One unresolved item was identified concerning improperly exiting LCO 3.4.6.2.f

when RHR isolation valve 1-FCV-74-1 had been declared inoperable. This matter

will be pursued during future inspections.

In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter, its enclosures,

and any reply will be placed in the NRC's Public Document Roem.

The responses directed by this letter and its enclosures are not subjected to

the clearance procedures of the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, PL 96-511.
