Tennessee Valley Authority Docket Nos. 50-327, 50-328 Sequoyah License Nos. OPR-77, DPR-79 I

i During the Nuclear Regulatory Commission (NRC) special heatup shift coverage inspection conducted September 25 - October 4,1988, a violation of NRC require-ments was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

l Technical Specification 6.8.1 requires that procedures recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, be established, imple -

4 mented and maintained. This includes administrative procedures.

Administrative Instruction 47, Conduct of Testing, requires that for l

testing interruptions extending more than one shift the test director I

Contrary to the cbove, Surveillance Instruction 94.4 was interrupted i

August 26, 1988, and was re-initiated September 25, 1988, and the test I

This resulted in a I

reactor trip signal being generated and the opening of reactor trip breaker "A".

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit a written statement or explanation to the Nuclear j

Regulatory Commission, ATTN:

Document Control Desk, Washington, DC 20555, with a copy to the Director, Office of Special Projects, and a copy to the NRC Resident Inspector, Sequoyah, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include (for each violation): (1) admission or i

denial of the violation, (2) the reason for the violation if admitted (3) the

corrective steps which have been taken and the results achieved, (4) the l

corrective steps which will be taken to avoid further violations, and (5) the date when full compliar.ce will be achieved. Where good cause is shown, con-sideration will be given to extending the response time.
