Inspection Report No. 50-219/86-12
This refers to the routine safety inspection by Messrs. W. Bateman and
J. Wechselberger from April 14 - June 1,1986 at the Oyster Creek Nuclear
Generating Station and the GPUN Corporat; offices in Parsippany, NJ and the
Region based inspection by Mr. W. Baunack from April 14-17, May 13-16, and
May 27-30, 1986 at the Oyster Creek Nuclear Generating Station. The inspec-
tions consisted of document reviews, interviews and observations of activities
as documented in the enclosed inspection report.
The findings were discussed
with Mr. John Barton and other members of your staff at the conclusion of the
inspection.
Apparent violations of NRC requirements are cited in Appendix A and categorized
under the NRC Enforcement Policy 10 CFR 2 Appendix C (49 FR 8583, March 8,1984).
A reply is required and should be prepared in accordance with Appendix A.
An additional violation, not identified in Appendix A, regarding two inoperable
safety related snubbers identified 'a significant concern in that the plant
operated for at least one complete operating cycle in violation of Technical
Specification limiting conditions for operations. This violation reflects in-
adequate restoration of a plant system upon completion of outage activities.
We will be in contact with you in the near future to schedule an enforcement
conference to discuss this matter. Subsequent enforcement action will be trans-
mitted under separate correspondence.
Overall, these violations highlight continuing problems in control of outage
work and in the implementation of the design change / modification program at
Oyster Creek and are apparently attributable to lack of attention to detail.

In many cases this represents a failure to implement procedures associated with
your design change / modification activities. Additional examples of these types
of problems were conveyed to you in previous inspection reports (50-219/86-06
dated May 12, 1986, and 50-219/85-35 dated February 14,1986). This inspection
report also identifies other design review related problems that require your

attention. Please be prepared to discuss your overall perspective of the
underlying causes and your plans to resolve these problems at the upcoming
'
enforcement conference.
