HIGHLIGHTS OF WEEKLY INFORMATION REPORT a

WEEK ENDING OCTOBER 17, 1986 Perry 1 The U.S. Court of Appeals (6th Circuit in Cincinnati) has lifted the Stay which prevented the Comission from voting on authorizing Perry 1 full power licensing, and dismissed the intervenor's Petition for review. The intervenor's Petition sought to introduce the January 1986 Ohio earthquake for litigation in the Perry proceeding, a petition previously denied by the Comission (CLI-86-07). Until the Court's opinion is received, it cannot be ascertained whether the Court's action finally settles the matter. It is possible that once the Comission votes, the intervenor will refile its Petition with the Court requesting to have the earthquake issue litigated at a hearing.

The licensee has revised his' schedule for Perry 1 readiness for full power licensing. It is now estimated that nuclear heatup testing will not be com-pleted until October 24, 1986, and that after a 10 day main-tenance outage, Perry 1 will be ready to exceed 5% of rated thermal power on/or about November 10, 1986.

Cooper Alert and Notification On October 10, 1986, FEMA provided findings to the NRC on the ade-quacy of the Alert and Notification System (ANS) for Ccoper Nuclear

, Station FEMA statire that it cannot make a statement of reasonable assurance that the Ccoper ANS is adequate to provide prompt alerting and notification to the public in the event of an emergency at the site. The issue basically involves notification of transients in a wildlife refuge, a state park, and river traffic.

Based on its finding, FEMA intends to withdraw its 44 CFR 350 finding of adequacy for the States of Nebraska and Missouri if after four

! months the states have not either corrected the deficiencies or submitted an acceptable plan for corrective action. NRC Headquarters and Region IV staff are evaluating the FEMA submittal.

Sequoyah Fuels Corpcration (SFC)

The findings of NRC inspections subsequent to the January 4,1986 accident demonstrate that SFC procedures were flagrantly violated, that the physical equipment and facilities used for filling and weighing UF cylinders were inadequate for safe use with 14-ton cylinders, gThat management failed to ensure that operating proce-dures important to safety were properly implemented, and that management failed to ensure that its personnel received adequate training in order to perform their duties. These findings demon-strate a najor breakdown in SFC's menagement control and oversight of ifcensed activities. The NRC finds that this failure of manage-ment was a contributing factor to the accident. The accident resulted in the death of an individual and demonstrates the need

for major inprovement in SFC's control and implementation cf its radiation safety program to ensure strict adherence to NRC require-ments and the safe performance of licensed activities.

To emphasize the seriousness with which the NRC views these viola-

, tions and the need for SFC to maintain an effective and aggressive managerent system to prevent such violations and their adverse l consequences to public health and safety, a Notice of Violation and

Proposed Imposition of Civil Penalties in the amount of Three Hundred Ten Thousand Dollars ($310,000) was issued to SFC.

In a separate action, the Director of the Office of Inspection and Enforcement rendered a Decision on four Petitions for action under 10 CFR Part 2.206. The Director determined that actions necessary to protect the public health and safety already have been taken and that additional actions requested by the Petitiers are not warranted.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR OCTOBER 11 - 19, 1986

1. DEFUELING

- The core drilling rig has been installed on the shielded work platform and the debris bed cleared of end fittings in preparation for drilling operations. The drilling operations are expected to begin on Monday, October 20, 1986.

2. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and The City of Lancaster Agreement.

- The Lancaster water semple taken at the water works river intake and analyzed by EPA consisted of a seven day ccmposite sample taken September 28 - October 4, 1986. A gamma scan detected no reactor related activity.

- TMI water samples taken by EPA at the plant discharge (includes Units 1 and 2) to the river consisted of seven daily composite samples taken from September 27 - October 4, 1986. A gamma scan detected no reactor related activity.

- The EPA analyses of the NRC outdoor air sample for the period October 9 - 16, 1986 showed that concentrations of Cs-137 and I-131 were below the lower limit of quantitative detection (LLD) for the system. The LLD varies around a value of IE-13 uCi/cc.

3. AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES

- Kelly vacuuming continues in the annulus area between the reactor building and fuel handling building. Work continues on the 281' and has been completed on the 305', 328', and 347' elevations.

- A two week campaign to flush the makeup and purification system piping was completed. The purpose of the flush was to reduce exterior dose rates around the piping.

- Scabbling has been started in the east corrider of the 328' elevation fuel handling building.

4. NRC EVALUATIONS IN PROGRESS

- Technical Specification Change Request numbers 49, 51, 52, and 54.

- Recovery Operations Plan Chr.nge numbers 31 and 33.

- Solid Waste Facility Technical Evaluation Report.

5. EVALUATIONS COMPLETED The NRC staff approved GPUN's Core Bore Operations Safety Evaluation Report (SER) on October 16, 1986. This permits expanded use of the core drilling rig as a defueling tool. The drill rig will be used to drill holes in the hard crust on top of the core. The drilling is limited to the area above the lower core grid and within a 48)" radius of core i center. Drilling operations are expected to last several weeks.
6. PUBLIC MEETING The next meeting of the Advisory Panel is tentatively scheduled for December 10, 1986. At that meeting the licensee will give a presentation on their licensing submittal for the Post Defueling Plant Status. That submittal is scheduled for issue to the NRC staff in November.

Office of Inspection and Enforcement Items of Interest Week Ending October 17, 1986

1. Items of Interest
1. Cooper Alert and Notification On October 10, 1986, FEMA provided findings to the NRC on the ade-quacy of the Alert and Notification System (ANS) for Cooper Nuclear Station FEMA statirr that it cannot make a statement of reasonable assurance that the Cenper ANS is adequate to provide prompt alerting and notification to the public in the event of an emergency at the site. The issue basically involves notification of transients in a wildlife refuge, a state park, and river traffic.

Based on its finding, FEMA intends to withdraw its 44 CFR 350 finding of adequacy for the States of Nebraska and Missouri if after four months the states have not either corrected the deficiencies or submitted an acceptable plan for corrective action. NRC Headquarters and Region IV staff are evaluating the FEMA submittal.

2. Oconee On October 1, 1986 while conducting tests on the Unit ? service water (LPSW) system, a new failure mode in the water supply was identified. The ultinate heat sink draws water via a siphon from the circulating water (CCW) intake structure. As a result of air inleakage, low lake level and the arrangement of the CCW water structure / piping, water could not be provided to the heat exchangers.

Subsequently, it has also been learned that the standy shutdown facility (SSF) also relies on the CCW system for its scurce of water. The loss of siphon also causes the SSF's auxiliary service water (ASW) pumps to fail. The SSF was installed to satisfy fire and I

security concerns and was intended to be independent of other l facility features.

3. Sequoyah Fuels Corpcration (SFC)

The findings of NRC inspections subsequent to the January 4, 1986 accident demonstrate that SFC procedures were flagrantly violated, that the physical equipment and facilities used for filling and I weighing UF g cylinders were inadequate for safe use with 14-ton "

To emphasize the seriousness with which the NRC views these viola-tions and the need for SFC to maintain an effective and aggressive management system to prevent such violations and their adverse consequences to public health and safety, a Notice of Violation and Proposed Imposition of Civil Penalties in the amount of Three Hundred Ten Thousand Dollars ($310,000) was issued tc SFC.

In a separate action, the Director of the Office of Inspection and Enforcement rendered a Decision on four Petitions for action under 10 CFR Part 2.206. The Director determined that actions necessary to protect the public health and safety already have been taken and that additional actions requested by the Petitions are not warranted.

4. Civil Penalties Paid Union Electric (Callaway) in the amount of $25,000 based on the failure of licensee personnel to recognize the requirements for a limiting condition for operation (LCO). (EA 86-119)
2. The following Significant Enforcement Actions were taken during the past week:
a. An Order Imposing Civil Monetary Penalty was issued to Nebraska P*lic Power District (Cooper Station) in the amount of $25,000.

Tnis action was based on a violation involving a degraded vital area barrier. (EN 86-24A)

b. An Order Imposing a Civil Monetary Penalty was issued to Florida Power and Light Company (Turkey Point) in the amount of $25,000.

This action was based on an unauthorized entry of a plant worker into a locked high radiation area. As a consequence of the licensee's extensive corrective actions, sufficient basis existed for partial nitigation. (EN86-25A)

c. An Order Imposing Civil Monetary Penalties was issued to Arizona Nuclear Power Project (Palo Verde) in the amount of $100,000. This action involved multiple security violations including access control and degraded vital area barriers and a failure to report. (EN 86-28A)
d. An Order Imposing Civil Monetary Penalties was issued to Combustion Engineering (Windsor, CT) in the amount of $15,000. This action was based on two violations which involved the unauthorized transfer of licensed material and use of licensed material at an unauthorized location. (EN 86-47A.).

e. A Notice of Violation and Proposed Imposition of Civil Penalty was issued to Portland General Electric Company (Trojan) in the amount of

$50,000. This action was based on two apparent violations of NRC requirements: failure to maintain operable flow paths for the RHR system for cold leg injection and incomplete inspection by a contract quality control inspector for the installation of a pressurizer safety valve. (EN 86-76)

f. A Notice of Violation and Proposed Imposition of Civil Penalties was issued to Sequoyah Fuels Corporation (Gore, OK). This action was based on the January 4,1986 accident in which an overfilled UF6 cylinder ruptured while it was being heated in a steam chest.

g. An Order Suspending License and Order to Show Cause (Effective Immediately) was issued to Advanced Medical Systems, Inc. (Geneva, OH). These actions were based on findings that employees were directed to perform certain service and maintenance on teletherapy equipment at medical facilities without NRC authorization, and without training, radiation detection and monitoring eouipment or required service manuals, and express objections to performing such maintenance without proper training (EA 86-155). OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending October 17, 1986 Nuclear Plant Analyzer A major feature of the Nuclear Plant Analyzer (NPA) is the ability to store and replay data from previously generated computer code calculations. As part of the NPA program at the INEL, a library of replays from reactor simulator code calculations has been assembled. The data from three key calculations were recently added to the NPA replay library. They are:

(a) A TRAC-PF1 calculation for a RESAR-3S reference plant undergoing a 4-inch cold leg break.

(b) A RELAP5/ MOD 2 calculation for Crystal River and Rancho Seco plants with upper head noncondensible gas venting.

(c) A RELAP5/ MOD 2 calculation of a main steam line break in the Arkansas Nuclear One - Unit Two (CE) plant.

The second issue of the NPA newsletter "NPA Update" featured the newly implemented replay files archiving procedures designed to reduce replay file storage costs significantly. Additional procedural changes will reduce NPA program storage costs on the order of $19 per day per user.

Based on informal records of analysis time required to perform accident studies with the use of the NPA features as compared to examination of raw computer cutput, experience has shown that a reduction of 8 to 25% of an analysts time is being realized with the NPA depending on the plant being modeled and the transient being analyzed. This improvement in analysts efficiency is attributed to the more direct medium of presentation of computed results through the NPA than was formerly available through static graphs and printouts.

Research on Effects of BWR Water Chemistry on Stress Corrosion Cracking l in Stainless Steel Piping A meeting was held on September 74 and 25 between representatives of NRC, NRC contractor, Argonne National Laboratory, and BWR Owners Group contractor, General Electric Co., to coordinate research on the effects of BWR water chemistry on stress corrosion cracking in stainless steel piping. - 2 chemistry developments including tests in operating reactors, tests on replacement materials Type 316NG and Type 347 stainless steel, weld overlay protection and decontamination. Plans were developed to resolve discrepancies in the results of tests on the cracking response of Type 316NG in BWR water environments.

Coordination meetings such as this are held periodically between ANL and GE to share results and coordinate ongoing work. On several occasions in the past, results of the NRC-ANL work have caused changes in the conclusions and direction of the GE work. ,

Commissioner Carr was not present when this item was affirmed. Accordingly, the formal vote of the Commission was 4-0 in favor of the decision.

Commissioner Carr, however, had previously indicated that he wotild approve this paper and had he been present he would have affirmed his prior vote.
