. , HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING JULY 25, 1986 San Onofre Nuclear Generating Station. Unit 1 (SONGS-1)

On July 16, 1986 Southern California Edison Company informed the SONGS Resident Inspector that the licensee's Safety Review Comittee had decided not to perform a planned test in which offsite power would be interrupted with the plant at 25% power. The licensee determined that the test would result in j overpressurization and failure of the turbine rupture discs if offsite power was not restored in 12 minutes allowing the circulating water pumps to provide condenser cooling water.

At the June 12, 1986 Comission meeting, the . licensee told the Connission of plans to perform these tests at 0% and 25% power. These plans were made voluntarily by the licensee and the NRR June 5, 1986 Restart Evaluation did 4

not require the perfonnance of these tests. The licensee has successfully performed the 0% power trip and verified electrical system performance by (1) auto-starting and manually loading the diesel generators, (2) verifying the performance of the semi-automatic delayed offsite power source, and (3) verifying the capability to transfer from the diesel generators to the delayed offsite power source without deenergizing the safety buses.

NRC Regulation of DOE Transportation Activities Under the Nuclear Waste Policy Act (NWPA)

DOE has recently transferred spent fuel to the Idaho National Engineering Laboratory (INEL) from other facilities for research purposes as provided under NWPA. As a policy decision, the Department of Energy (DOE) has made the shipments in NRC-certified packages. The transfers have focused attention on whether DOE shipments under NWPA are subject to all NRC's transport regulations.

In response to an NMSS request for an opinion on this matter, OGC has basically advised that DOE is not required by NWPA (or other law) to comply with NRC's transport regulations for spent fuel transfer. OGC also advises that, except for possible shipment under the NWPA interim storage program, other DOE NWPA shipments would similarly be exempt from NRC regulation.

Severe Accident Risk Assessment The RES staff and supporting contractors are currently engaged in the reevalua-1 tion of the risk of six reference LWRs. This reevaluation is currently being used to support NRR's review of IDCOR submittals. In addition, it will support the staff report NUREG-1150, which is intended to provide guidance to decision-makers as to the appropriate role of information on core melt frequency, containment performance, source terms, and risk in making severe accident decisions.

) A meeting was held on July 14-15 in Albuquerque, New Mexico, to discuss progress. The meeting was attended by representatives of laboratory management at Sandia National Laboratories, Battelle Columbus Division. Oak Ridge National Laboratory, Brookhaven National Laboratory, and Idaho National Engineering Laboratory. The laboratory managers generally felt the work done to date was

! impressive, had several suggestions for improvement that are being evaluated, and expressed concern regarding publication of draft NUREG-1150 for comment by September 30, 1986.

ADMINISTRATIVE MATTER On June 6, 1986, the Contracting Officer requested the Small Business Administration (SBA) to determine the eligibility of a proposal received in response to Solicitation No. RS-0RM-86-264, a competitive requirement set-aside exclusively for participation of small business. The offeror appeared to violate the spirit and intent of the small business set-aside program by proposing extensive subcontracting, including the project management function, to la.'ge business firms. On July 11, 1986, SBA determined that the proposed arrangement constitutes a joint venture which creates a business entity that exceeds the size standard under this solicitation. Therefore, the offeror does not qualify as a small business for this acquisition. The SBA notified the offeror of this determination on July 21, 1986.

0FFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending July 25, 1986 F

River Bend Station On July 17, 1986, the licensee re carted River Bend Station after a 23 day outage. The outage was initia+ after the completion of the River Bend Station startup test prograr niver Bend Station experienced their 18th and 19 unanticipated scrams on July 8 and 13, 1986 during earlier attempts to restart from this outage.

On July 8,1986, River Bend Station scramed from 65% power following a main steamline isolation valve (MSIV) closure due to a steam leak through a failed gasket on a moisture separator reheater drain tank. As a result of the steam leak, personnel were evacuated from the turbine building --no injuries occurred.

The licensee evaluated the root cause of the scram and reexamined all related maintenance work requests (MWRs) perfomed during the last two outages.

On July 13, 1986, River Bend Station scramed from 55% power due to a loss of condensor vacuum which resulted in a turbine trip. The imediate cause of the turbine trip was a loss of steam to the turbine seals with operator response to an alarm a contributing factor. In response, the licensee reviewed all i balance of plant MWRs, control room annunicators and pending modification requests, as well as recalibrating trip setpoints in the seal water tank and instructing their operators on appropriate procedures.

The staff is monitoring the licensee's follow up closely. The project manager

, has formed a River Bend Oversight Group with representation from NRR, IE and Region IV and the senior resident inspector.

Arkansas Nuclear One, Unit 1 I Arkansas Power and Light Company has been inspecting the Limitorque valve operators for AN0-2 during the current ANO-2 refueling outage. Of 81 valve operators the licensee found that 40 had internal wiring connec-tions and wires which were not environmentally qualified. The licensee is in the process of replacing the non-environmental wiring components in the valve operators of ANO-2. It takes 4-5 hours for each valve to do this. An equipment qualification inspection of ANO-1&2 is in progress and the staff is concerned about how this discciery affects Unit 1 which is operating at about 76% power and is scheduled for refueling shutdown August 20, 1986. The licensee has recognized that the potential for such a problem existed since November 1985 and elected to investigate it during the current and upcoming refueling outages.

Since some valves, including valves required to be operable by the Technical Specifications could be declared inoperable, the Inspection Team has requested the licensee to provide justification for continued operation (JCO).

j Limerick On May, 17, 1986, the ASLAB reversed and remanded the ASLB's Third Partial Initial Decision on one issue - school bus driver availability to drive buses _ ,

to evacuate school children in response to an emergency at the Limerick  ;

Generating Station. l The current status of this issue, as discussed in a teleconference among the parties on July 18, 1986, is that a hearing will be held on August 18 and, if needed, on August 22, 1986. Witnesses will be provided by PECo, the Pennsylvania Emergency Management Agency, and the Federal Emergency Management Agency. The intervenor, Limerick Ecology Action, may subpoena several involved school district officials and will also question some of the school bus drivers ,

to be provided by PECo. - s Final Findings are dua from all parties by August 26 or August 29 if the August 22 hearing date is needed. As of July 18 Unit 1 is operating at full power and Unit 2 is actively under construction. -

Rancho Seco Nuclear Generating Station About 7:45 PDT on July 21, 1986, seismic activity near Mamouth Lakes, California (about 15 miles north of Bishop, California) was felt by personnel at Rancho Seco. The seismic response at Rancho Seco was below the threshold for initiation of the site seismic instrumentation.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR JULY 21 - JULY 27,1986

1. DEFUELING

- The licensee continued core boring to acquire core stratification samples in positions D-4, 0-7, and 0-9. Encased stratification samples were removed and placed in defueling canisters for eventual shipment to Idaho National Engineering Laboratories. Video inspections were made through the access ports left by the drilling operations. The licensee has concluded core stratification sampling. A total of 10 sampling penetrations have been made.

Preparations are being made to systematically drill the core using a solid face bit. Such drilling is designed to facilitate bulk defueling by aiding in size reduction of fused core debris.

2. SHIPMENT

- The first fuel shipping cask arrived at Idaho National Engineering Laboratory. There was a brief delay in route prior to entering the State of Nebraska in response to concerns by that State's governmental officials. Upon resolution to the concerns, the balance of the trip was uneventful.

3. PLANT STATUS

- The reactor remains in long term cold shutdown, vented to the atmosphere. Core cooling is by natural heat loss to ambient building atmosphere. The average incore thermocouple reading is 81*F.

- The airborne radioactivity on the defueling platform is about 6.2 E-7 uCi/cc Tritium and 1.6 E-11 uC1/cc particulates, predominately Cesium-137 and Strontium-90. The platform is mounted above the modified internals indexing fixture which is mounted over the reactor vessel. These provide about 15 feet of water over the core region and 6 feet over the carousel holding the defueling canisters.

4. WASTE MANAGEMENT

- The Submerged Demineralizer System (SDS) completed processing batch S-135, 19,603 gallons from the neutralizer tank.

- EPICOR II completed processing batch 302, 1,580 gallons from j WDL-T-118 and batch 303, 23,312 gallons from CC-T-1.

- Total volume processed through SDS to date is 4,084,856 gallons, and the total volume processed EPICOR II to date is 3,055,238 gallons.

5. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and The City of Lancaster Agreement.

- The Lancaster water sample taken at the water works river intake and analyzed by EPA consisted of a seven day composite sample taken July 6 - 12, 1986. A gamma scan detected rio activity that could be attributed to TMI-2.

- TMI water samples taken by EPA at the plant discharge (includes Units 1 and 2) to the river consisted of seven daily composite samples taken from July 5 - 12, 1986. A gamma scan detected no activity that could be attributed to TMI-2.

- The EPA analysis of the NRC outdoor air sample for the period July 16 - 24, 1986 showed that concentrations of Cs-137 and I-131 were below the lower limit of quantitative detectability for the system.

- On July 24, 1986 the NRC staff approved a portion of the defueling activities described in GPUN's Defueling Safety Evaluation Report, Revision 10. This includes bulk defueling activities above the grid support structure and the use of solid face bit drilling operations in non-instrumented core locations using the core drilling rig. The remainder of the above Safety Evaluation Report is still under review.

8. PUBLIC MEETING The Advisory Panel for the Decontamination of Three Mile Island Unit 2 will meet on August 13, 1986 from 7:00 PM to 10:00 PM at the Lancaster Council Chambers, Public Safety Building, 201 North Duke Street, Lancaster, PA. At the meeting the panel will receive a status report on the progrets of defueling from GPU Nuclear and a briefing on the licensee's plans for disposal of the processed water generated from the accident.

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending July 25, 1986 Shipping Cask for THI-2 Core Debris - Update An IE inspection of the fabrication of the Model No. 125-8 shipping cask to be used to tvansport the canisters containing TMI-2 core debris noted a previously unidentified inclusion in the 2-inch thick outer shell. The inclusion was classified as incomplete penetration.

The radiographs and enhanced radiographs were reviewed by three NRC consultants. The consensus of the three examiners is that the linear indication is slag. The linear length of the slag would be acceptable under the ASME code and the design that has been approved.

On July 17, 1983, the Certificate of Compliance for the Model No. 125-8 was amended to incorporate modifications requested by the Department of Energy.

The first DOE shipment of core debris using the NRC-certified package left TMI by rail on Sunday, July 20.

On Tuesday evening, July 22, at the request of the Governor of Nebraska's Office, Union Pacific detained the shipment near the Kansas border prior to entry into Nebraska.

Nebraska officials met with DOE officials aboard the train. The shipment was allowed to proceed after a delay of about 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br />.

NRC Regulation of DOE Transportation Activities Under the Nuclear Waste Policy Act (NWPA)

DOE has recently transferred spent fuel to the Idaho National Engineering Laboratory (INEL) from other facilities for research purposes as provided under NWPA. As a policy decision, the Department of Energy (00E) has made the shipments in NRC-certified packages. The transfers have focused attention on whether DOE shipments under NWPA are subject to all NRC's transport regulations.

In response to an NMSS request for an opinion on this matter, OGC has basically advised that DOE is not required by NWPA (or other law) to comply with NRC's transport regulations for spent fuel transfer. OGC also advises that, except for possible shipment under the NWPA interim storage program, other DOE NWPA shipments would similarly be exempt from NRC regulation.

DOE OCRWM/ Defense Programs Subaareement on Defense Waste Shipments ,

1 DOE's Office of Civilian Radioactive Waste Management (OCRWM) and Defense  !

Programs (DP) recently concluded a subagreement to their Memorandum of Agreement on Transportation Programs. The subagreement essentially designates OCRWM as the office responsible for shipments of Defense Programs high-level ,

waste to a civilian radioactive waste repository. The subagreement specifies that DP will load the waste into NRC certified packages to be provided by OCRWM.

Sequoyah Fuels Contingency Plan On July 16, 1986, representatives of Region IV, the Office of Nuclear Material Safety and Safeguards, the Environmental Protection Agency (EPA), and the Federal Emergency Management Agency (FEMA) met to discuss their areas of responsibility and status in reviewing the Sequoyah Plan and local / county / state planning efforts. The Nuclear Regulatory Commission is coordinating with EPA and FEMA in reviewing the Sequoyah Plan and procedures for compliance with each agency's requirements.

Livermore National Laboratory Nuclear Criticality Information System (NCIS)

On July 21-22, 1986, Mr. Brian L. Koponen, Project Leader, Lawrence Livermore National Laboratory Nuclear Criticality Information System (NCIS) conducted training sessions with members of the Urinium Fuel Licensing Branch and Transportation Certification Branch on the app'ication and use of the NCIS.

The NCIS is a vital part of the Department of Energy's (DOE) criticality safety program. It offers electronic communications, interactive database management, and bibliographic, textual, and numeric resources to a growing community of users in the field of nuclear criticality safety. Although NCIS users presently represent DOE headquarters, national laboratories, DOE contractors, and NMSS nuclear criticality safety personnel, plans are being made to extend its use to private industry and the international community.

Planned Restart Inspection of Sequoyah Fuels Corporation, UF, Facility, Gore, Oklahoma During the week of July 28, 1986, Region IV representatives will conduct a restart inspection of the Sequoyah Fuels Corporation UFg production facility.

4 Several open items generated from findings of the January 1986 inspection remain to be closed out. Representatives of NMSS, FEMA, and EPA will participate in this inspection.

.3-Status of Hearings

1. Sequoyah Fuels, Gore, Oklahoma, UF , to UF 6 4 Administrative Judge Frye has scheduled a meeting of all parties to the informal hearing on August 1, 1986, in Sallisaw, Oklahoma. The informal hearing has now been tentatively rescheduled for the week of August 11, 1986.

However, by memorandum dated July 3, 1986, Judge Frye recommended to the Commission the adoption of formal hearing procedures. Action by the Ccmmission on these recommendations may impact the schedule for the hearing.

2. Sequoyah Fuels, Gore, Cklahoma, Comprehensive Waste Disposal Plan The hearing is being held in abeyance pending submittal of a new plan as part of the revision to the decommissioning plan required by Source Material License No. SUB-1010, Condition No. 21.
3. Kerr-McGee, West Chicago, Illinois, Kress Creek Decontamination The ASLB decision dated June 19, 1986, dismissed the Order to Show Cause issued to Kerr-McGee Chemical Corporation on March 2, 1984. On June 30, 1986, the staff filed a notice of appeal with ASLAB.
4. Kerr-McGee Rare Earths Facility, Decommissioning Plan, West Chicago, Illinois This hearing is being held in abeyance pending the staff's completion of the supplement to the Environmental Impact Statement. The current schedule calls for issuance of the Draft Supplement by October 1,1986, and the Final by May 1, 1987. The Board has indicated that they may resume a portion of the Hearing after the Draft is issued.

Information Notice Issued on Increased Monitoring for Patients with Certain Nuclear-Powered Pacemakers On July 14, 1986, IE Information Notice No. 86-59 was sent to all NRC licensees authorized to use nuclear powered cardiac pacemakers. The notice was to alert licensees that, by letter dated March 14, 1986, Coratomic, Inc.,

a nuclear pacemaker manufacturer, has recommended increased monitoring of patients with certain Coratomic units implanted. The units in question cre Models C-100 and C-101 manufactured between September 1974 and October 1977.

Coratomic, Inc. said that "2CN potting material" (a glue-like substance) used in these units reverted to a viscous form leading to a rate drop and a higher-than-anticipated random transformer failure rate.

l 0FFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending July 25, 1986 NRC Employee Honored The Health Physics Society recently bestowed its Founders Award upon Dr. Allen Brodsky of the NRC Office of Nuclear Regulatory Research. This professional society, whose members specialize in the protection of man from the harmful effects of radiation, has more than 6,000 members throughout the U.S. and abroad. The Founders Award is presented to distinguished scientists who have made outstanding contributions in the field of radiation protection.

There have been only 8 previous recipients since the society was founded in 1957. Dr. Brodsky has received wide recognition for his publications in this field, including an unusually large number of highly useful regulatory guides and NUREG reports published during his 16 years with the AEC/NRC. Dr. Brodsky, who has a total of 22 years of government service, retireed in July 1986.

l RES RULEMAKING ACTIVITIES AND OTHER RELATED ISSUES Emergency Preparedness for Fuel Cycle and Other Radioactive Materials Licensees (Parts 30, 40, 70)

In 1981 the NRC issued orders to certain fuel cycle and other radioactive material licensees requiring them to submit comprehensive onsite radiological emergency plans. Subsequently, rulemaking was initiated to place a requirement for emergency plans in the regulations.

The proposed rule was sent to the Commission on March 25, 1986 (SECY-86-99),

but the recommendation was that the Commission withhold action pending further information on the NRC's authority to regulate the chemical toxicity of licensed materials and nonlicensed materials at licensed facilities. This information has now been provided. A memo, from the EDO to the Office of the Secretary, transmitting revised pages concerning the Sequoyah accident " Lessons Learned Report" and recommending approval of the Federal Register notice, was sent on July 22.

Severe Accident Risk Assessment The RES staff and supporting contractors are currently engaged in the reevalua-tion of the risk of six reference LWRs. This reevaluation is currently being used to support NRR's review of IDCOR submittals. In addition, it will support the staff report NUREG-1150, which is intended to provide guidance to decision-makers as to the appropriate role of information on core melt frequency, containment performance, source terms, and risk in making severe accident decisions.

A meeting was held on July 14-15 in Albuquerque, New Mexico, to discuss progress. The meeting was attended by representatives of laboratory management at Sandia National Laboratories, Battelle Columbus Division, Oak Ridge National Laboratory, Brookhaven National Laboratory, and Idaho National Engineering Laboratory. The laboratory managers generally felt the work done to date was impressive, had several suggestions for improvement that are being evaluated, and expressed concern regarding publication of draft NUREG-1150 for comment by September 30, 1986.

Most of the premium rates for the Federal Employees' Group Life Insurance (FEGLI) have been lowered while none have been increased. The premium rate for the basic insurance has been lowered. The new rates become effective pay period 17/86 (August 3-16, 1986) for salary checks paid on August 26, 1986.

An announcement will be issued to advise all employees of the changes.

Walk-In Travel Voucher Processing A walk-in service for the audit and payment of certain types of travel vouchers will be implemented by September 30, 1986. An employee will be allowed to obtain cash payment, up to $500, on any amount due upon presentation of an approved temporary duty travel voucher incident to travel commencing on or after July 1,1986 within the continental U.S. (excluding Alaska). The service will be implemented August 4, 1986, for MNBB employees in order to fully test the procedures. It will then be implemented at each region before being implemented for the rest of the Headquarters' buildings.
