SUBJECT:

NOTICE OF VIOLATION AND NRC INSPECTION REPORT NO. 50-302/97-12

Dear Mr. Anderson:

An NRC inspection was conducted October 20 - 24, 1997. December 8 - 12. 1997,

and January 5 - 9. 1998, at your Crystal River facility. This was an

Emergency Operating Procedures (EOP) Inspection.

The enclosed report presents

the results of that inspection.

Based on the results of the inspection, the NRC has determined that violations

of NRC requirements occurred.

Violations w2re identified for failure to

comply with the 1984 Confirmatory Order associated with the preparation of

symptom based E0Ps. failure to take adequate corrective actions in the areas

of calculations and radiological doses to personnel performing E0P actions

outside the control room, performing calculations inconsistent with ANSI

45.2.11Property "ANSI code" (as page type) with input value "ANSI</br></br>45.2.11" contains invalid characters or is incomplete and therefore can cause unexpected results during a query or annotation process., 1974, and, failure to purchase the High Pressure Injection pumps to

appropriate technical requirements.

These violations are cited in the enclosed Notice of Violation, and the

circumstances surrounding the violations are described in detail in the

enclosed report.

Please note that you are required to respond to this letter

and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure compliance

.t th regulatory requirements.

The enclosed Inspection Report also identifies activities that violated NRC

requirements that will not be subject to enforcement action oecause the

licensee's efforts in identifying and correcting the violations met the

criteria specified in Section VII.B1 of the Enforcement Policy.

Therefore, no

response with respect to these matters is required.

There were three unresolved items identified in the enclosed report dealing

with a 1998 safety evaluation performed to support an Updated Safety Analysis

during the period of January 26 through 30. 1998, and found acce) table for

restart. In accordance with 10 CFR 2.790 of the NRC's " Rules of practice." a
