III. DEFINTTTONS A. VT Visual examinations to determine the condition of the part, components, or surface examined, including such conditions as cracks, wear, corrosion, erosion or physical damage on the surfaces or the part or components.

1. Direct VT Visual examination conducted when access is sufficient to place the eye within 24 inches (610 mm) of the surface to be examined and at an angle not less than 30 degrees to the surface. Hirrors may be used to improve the angle *

. of vision (see Figure 1).

2. Remote VT Visual examinations that may be substituted for direct VT-1 or when access or practicality is insufficient to place the eye within 24 inches (610 mm) of the surface to be examined and at an angle not less than 30 degrees to the surface (sco Figure 1),

a) Remote examination may use aids, such as telescopes, borescopes, fiber optics cameras, or other suitable instruments, provided such

TITLE:

VISUAL EXAMINATION PROCEDURE TO DETERMINE THE CONDITION OF NUCLEAR PARTS, COMPONENTS OR SURFACES III. B. Discontinuity - an interruption in a normal physical structure which may not affect the use of a component for its intended purpose.

C. Defect - a discontinuity which affects the usefulness of a component.

IV. EESB2HEIBILITIts A. The LMT General Manager, is responsible for the generation, control, and approval of this; procedure.

1. The approval of the General Manager shall be indicated by a dated signature on Page 1 of this procedure.

B. The LMT Technical Secretary, is responsible for

  • ccheduling personnel for certification and/or recertification, in accordance with the requirenents of this procedure.

C. The LMT responsible Field Supervisor, shall qualify the procedure for a particular examination.

V. PROCEDURE OUALIFICATIQH A. The procedure shall be qualified for specific examination, personnel, and equipment by succ,essful application on a similar component part, or device.

B. This procedure need only be qualified once for each application.

'VI. PERSONNEL REOUTREMENTS A. Personnel shall be qualified and certified according to the requirements of ASME Section XI, 1989 edition and meet the guidelines of SNT-TC-1A, August 1984 edition.

TITLE: VISUAL EXAMINATION PROCEDURE TO DETERMINE THE CONDITION OF NUCLEAR PARTS, COMPONENTS OR SURFACES VI. B. Level I, II, and III individuals must successfully complete the following physical requirements annually: >

1. An examination shall be conducted to assure natural or corrected near distance acuity, in at least one eye, an determined by the ability to ,

read the Jaeger No. 1 on a standard Jaeger test at not less than 12 incher or a near distance test pattern equivalent to a Snellen fraction of 20/20.

C. Personnel shall receive a color vision test to verify the distinction of cciors and the differentiating contrast between colors normally used in the applicable examination (s).

D. Only Level II or III individuals are permitted to investigate, interpret, or make final determination on the reportability of indications.

VII. EOUTPMENT AND MATERTAL REOUIREMENTS A. Commercially available equipment such as lences,'

cameras, lamps, mirrors, borescopes, fiberscopes, '

binoculars, calipers, micrometers, rulers, or special gages shall be used as necessary. Special o't remote equipment used shall be identified on the examination report.

a) Stress risers; b) Cracks; c) Burrs or nicks; d) Galling; e) Erosion; f) Corrosion; g) Wear; h) Physical Damage; i) Unusual stains; j) Blistered or chipped paint; k) Other discontinuities required by the cwner.

XII. EVALUATlQH A. All results shall be documented on the examination report (see Figure 12). Reporting should includh, where applicable, the following items:

2. The procedure and procedure revision number;
3. Evidence of unusual conditions previousl,y listed;
4. Weld configuration (see Figure 13);
5. Surface condition; .
6. Attachment and sketches as necessary;
7. Lighting used;
8. 10% neutral gray card results;
9. Remote or special equipment;
10. Drawing number (s).

B. The visual report shall state either "NRI" (no reportable indications) or the existence of unusual conditions previously listed.

TITLE: VISUAL EXAMINATION PROCEDURE TO DETERMINE THE CONDITION OF NUCLEAR PARTS, COMPONENTS OR SUk? ACES XII. C. For reportable indications, a reference system shall be a sketch of the item showing the relative location orientation.

D. Components whose visual examination detects the relevant conditions described in the standards of ASME Tables IWB-3410-1 and IWC-3410-1 shall be unacceptable for service.

1. Reportable indications may be verified by supplemental surface or volumetri examinations.

E. The final evaluation is subject to review by an LMT Level III, VT-1.

F. All reportable indications shall be reported to the client or his cognizant representative as soon as practical following the examination (s).

XIII. RECORDS -

A. An examination report is required for each examination, or defined assignment, uch as a portion of a system.

B. Photography may be 3.dded to the record at the option of

_ the examiner or the request of the client.

XIV. REVIEW A. Examination reports shall be subject to review by an assigned LMT' Level III Supervisor for conformance to the requirements of this pencedure.

XV. STORAGE AND DISTRT83UTION A. Examination. documentation shall become the property of the client upon his sign-off. Additional reports which may include examination documentation as referenced natorial shall be generated from copics.
