- DCR SAFsTY EVALUATION 10 CFR 50.59 2EC-1501 This design change improves the plant safety by making vital power available much sooner during ce.rtain postulated fire plus blackout conditions.
Failure of the control transfer switch may cause the ' associated diesel to fail to start during a safeguards actuation. This failure is considered to be within the realm of the single failure criteria. This modification does not alter any plant process or discharge. Therefore, no unreviewed safety or environmental questions are involved.
2EC-1566 This design change does not jeopardize the pressure retaining boundary nor impact the operation or basic design criteria of the Chilled Water System. This modification does not alter any plant process or discharge. Therefore, no unreviewed safety or environmental questions are involved.
2EC-1785 This design change replaced existing components with ones manufactured by a different vendor.
The replacement components have been seismically qualified to IEEE 344 which is to the same or higher stress levels as the original equipment.
This modification does not alter any plant process or discharge. Therefore, no unreviewed safety or environmental questions are involved.
2EC-1970 All realistic failure modes have been considered but are not applicable.
1 This design change meets GDC 56 requirements and complies with safety Guide #11. The additional equipment meets the design criteria of seismic I and nuclear class II. This modification does not alter any plant process or discharge. Therefore, no unreviewed safety or environmental questions are involved.

2EC-2000 The existing transmitters malfunction. The replacement ones are of better design. There is no functional change in the system or component.
This modification does not alter any plant process or discharge. Therefore, no unreviewed safety or environmental questions are involved.
2SC-1168 All potential, realistic failure modes have been considered but are not applicable. Regulatory requirements do nce apply. The replacement transmitters do not modify the function of the system. The original transmitters are considered obsolete by the manufacturer. This modification does not alter any plant process or discharge.
Therefore, no unreviewed safety or environmental questions are involved.

SALEM GENERATING STATION MONTHLY OPERATING
SUMMARY
- UNIT NO. 2 MAY 1986 SALEM UNIT NO. 2 The Unit began the period operating at full power. On May 2, 1986 at 1717 hours0.0199 days
0.477 hours
0.00284 weeks
6.533185e-4 months
, a controlled shutdown was initiated to perform
, environmental equipment qualification inspections in the Containment.
As the Unit was taken off the line, on May 3, 1986 at 0332 hours0.00384 days
0.0922 hours
5.489418e-4 weeks
1.26326e-4 months
, a Reactor Trip with Safety Injection occurred due to "High Steam Line Flow With Low Tave" which was apparently caused by a steam flow spike when the Generator output breakers were opened. On May 3, 1986 at 2231 hours0.0258 days
0.62 hours
0.00369 weeks
8.488955e-4 months
, the Turbine was latched and power ascension to full power operations was initiated. Unit power was reduced to 65% on May 11, 4
1986 at 0220 hours0.00255 days
0.0611 hours
3.637566e-4 weeks
8.371e-5 months
due to limited system load demand. The Unit was returned to full power the same day at 0642 hours0.00743 days
0.178 hours
0.00106 weeks
2.44281e-4 months
. During the period from May 23, 1986 to May 25, 1986 minor load reductions were required due to high river grasses and debris causing Circulating Water System problems. On May 31, 1986 at 2200 hours0.0255 days
0.611 hours
0.00364 weeks
8.371e-4 months
a load reduction to 61% was commenced in order to remove #21 SGFP from service for replacement of the governor. On June 1, 1986 at 1515 hours0.0175 days
0.421 hours
0.0025 weeks
5.764575e-4 months
, work on #21 SGFP was completed and the Unit was returned to 100% power where it remained for the rest of the period.
