ENCLOSURE 1 NOTICE OF VIOLATION Georgia Power Company Docket No. 50-424 Vogtle Unit 1 License No. CPPR-108 The following violations were identified during an inspection conducted on April 2 - May 13, 1986. The Severity Levels were assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).

1. 10 CFR 50, Appendix B, Criterion V and the accepted QA program (PSAR Section 17), requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instruutions, procedures, or drawings.

Contrary to the above, activities affee ,ing quality were not prescribed in that on April 9,1986, the Residual Heat Removal System (RHRS) was being filled and vented in support of system operation to obtain a Reactor Coolant System (RCS) sample without the use of appropriate procedures to control the evolution. Failure to have the appropriate procedures resulted in an improper valve lineup which allowed the Refueling Water Storage Tank to gravity fill the RCS. Approximately 40,000 gallons of water was added to the RCS (i.e., the level rose from the 188' elevation (mid-nozzle) to the 226' elevation 16' above the containment operating deck) before the operator took action to isolate the source of water.

This is a Severity Level IV violation (Supplement II).

2. 10 CFR 50, Appendix B, Criterion V, as implemented by FSAR, Chapter 17, Section 17.1.5, requires, in part, that procedures shall outline the manner in which work is conducted. Specification No. X3AR01, Section E8.7.1, paragraph 5 which covers raceway installation states " cable trays shall not be used as walkways or working platforms, where trays are so located that such use is possible or might be attempted, it shall be prohibited by clear and conspicuous signs and prevented, if necessary, by suitable physical barriers."

Contrary to the above, on April 17, 1986, cable tray Section 1BE502 TRAA was found damaged as evidenced by the distorted shape of the bottom and sides.

It is also being used as some type of walkway, evidenced by the fact that the cables are compressed into the tray bottom openings. This tray is located such that it should have been protected by one of the methods specified.

This is a Severity Level IV violation (Supplement II).

Georgia Power Company Docket No. 50-424 Vogtle Unit 1 2 License No. CPPR-108 Pursuant to 10 CFR 2.201, you are required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.

Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.
