4.5 Contamination wipe test - Perform o rodlooctlue contamination wipe test of the shipping pot.koge. This consists of rubbing filter paper or other absorbent material, using moderate pressure, over on area of 100 square centlineters of the package surface. The ociluity on the filter paper should not onceed 0.01 microcuries of removable contamination.

4.6 Inspection - Industrial Nuclear Company's Enposure Device, Model Ifi-100,1990 0 Package, is shipped in a Protectlpe Duerpack (steel drum),

USO/9105/B(U)(shipping container). Inspect the shipping container and the Euposure Device for signs of any demoge. If demoge is evident, the corrier and Industrial Nuclear Company should be informed immediately.

4.7 Documentation - The source (isotope), activity, model number, serial number, date received, rodletion levels and the Euposure Device (Model 111-100) and serial number should be recorded on a receiving report, llecords ore reuulted to be molntained for inspection.

10.1.4 Component Test - The lock assembly of the Model ll1-100 [Hposure Devl(e is Irispetted and tested to assure that the security of the sourte assembly mill be molntained, f ailure of this test mill plevent use of the potkage until the lock assernbly is replaced or repaired, inspected and

telested, ulnrning: The Model lll-100 Euposure Device has been prepared for use with Industrial Nuclear's sourto models, llomevet comparable sources from another rnonufacturet should be otteptable. No attempt should be inade to alter the lock body assembly or to insert on unauthorized source model.

10.1.5 Test for Shielding integrity - The rodlotion levels at the surface of the Model 111-100 luposure Device, and at I meter t 10"l from the surf ace ute measured using a calibrated survey instrument. The rodlotion levels, tuhen loaded to its rated tapacity, should not onceed 200 mil /hr at the surf ace nor 10 mil /hr at I meter HO*l from the surfore, failure of this test will prepetit shipinent uf the potkoge.

10.2 Maintenance Program - lhe Model ll1-100 Euposure Device is surveyed.

Inspetted and tested prior to the sole of the equipment.

D19ANMEMBLY OF Tile LOCK

( A) Remove drive cable receiver (Retainer).

(I) With a 5/fA allen wrench remove the 3 set screws of the drive cable receive items 8 & 9.

_(2) lloiding the safety Ltch plate in the operate puition remove the retainer, spring, slide receiver, aral dummy pigtail.

Items 11,12,13, & 15.

(B)- Remove lock cylinder and safely latch plate.

(I) With a 3/32 allen wrench renunc the screw urnier the lock cylinder, item 10.

-(2) With a long 3/32 allen wrench remove the allen screw in the kick tody. Item 3. Note: The screw is inside the housing, umlet the lock cylinder and is accexrible only from below. The screw (Item 10) must first be removed.

(3) *lhe safety latch plate is spring kaaded, lloid down lightly and with the key in the kick slightly rotaled, remove the kick cylinder assembly, item 2.

(4) Remove the safety latch spring aad pin. Items 4,5, & 6.

HP AMNPMHLY OF Tile LOCK

( A) Install nindifkd safety latch plate, spring and pin, (I) Place eximsure device on its side so that the expuure device handle is on the right. Install pin, spring and nxxlified safety latch plate in the kick lxxty, items 4,5, & 6. Note: Safety latch plate must be installed with the added spacer plate next to the exposure device tuly.

(11) Install lock cylinder in body.

(1) Lock cylinder must be insta: led in the unkicked guition with hole in tottom of cylinder down to a ign with allen screw installed through bottom of kick txxty, item 2.

(2) Lock cylinder must be properly inserted in lock tuly in order for cam to engage safety latch plate. Excessive insertion will misalign hole with allen screw and may inhibit safety lavh plate action.

(3) Secure kick cylinder with allen screw, item 3.

(4) Lock cylinder in the unkicked position. Test safety latch plate for snumth up and down travel.

(C) install drive cable receiver.

(1) Depress and hold down the safety latch plate (operate position) and insert the slide receiver. Item 13. Note: The pin projecting from the receiver slide must be aligned so as to enter the smail hole inside the k)ck tely casily and the safety latch plate. Correct rotation may be verified by observing that the index n' ark on the face of the receiver is at the tuttom ami that the safety latch plate remains in the operate position.

(2) Insert the large spring in the slide receiver, item 12.

(3) Fully insert drive cable receiver / retainer into kick luly cavity and rotate until retainer screw holes align with holes in the sides of the lock toly. Item 11. Insert allen ser screw in imitom of kick tuly to hold retainer, item 8.

(D) Test for proper assenibly.

(11 Observe that the safety latch plate opening matches the hole through the slide receiver (operate position).

Push drive cable thmugh exposure device and attach dummy pig tail to drive cable.

Pull drive cable with dummy pig tail back (retract) through exposure device. At this [ mint, the safety latch plate should pop up to the latched position. Test for proper action by depressing safety latch plate, push dummy pig tail out and return. With safety latch plate in up position and dmnmy pig tail trap 5 al rotate key to k>ck puition. Pull cable back,

- rotate key to unkick. Finally, to verif f

/ or proper assembly, depression of the safety latch plate to the operate position will pennit removal of the pig tail through the outlet port of the exposure device.

(2) Complete in.mlion of allen screws, Item 8 & 9, described in #7 (above) and kick side screws with special short screws.

Replace screw uraler tie kick cylinder. Item 10.

(E) Test complete systein as follows:

(l) Pull back (retract) dummy pig tail.

(2) Safety latch plate will pop up.

(3) Push out (expose) dummy pig tail will not move out of exposure device.

(4) Rotate key to kick position and remove Dummy pig tail will not move out of exposure device. Safety !atch plate cammt be dern:ssed.

(5) hacrt key and attempt to rotate. (Do Not Force) Key should not rotate to unkick pnition.

(6) Pull back (retract) dummy pig tail and rotate key to unlock position. Safety latch plate will depress slightly as key turns.

(7) Release pull on dummy pig tail, key in unkick position safety latch plate may be manually depressed to operate guiuon.

(M)

Rotate key to kick usition. Key should not rotate.

l (9) Remove dummy pig tail. Exposure device is now ready for re-kniing.

ALL IR 100 OWNERS / RSOs Industrial Nuclear Company (INC) recently received information that the safety latch plate of the IR 100 !!xposure Device would bend when the safety latch plate was struck directly on the face. We had performed the horizontal shock test on the lock box in 1991, as requhed by ANSI N432-1980, Section 8.2 and tbc IR 100 passed without difficulty, The lit 100 Exposure Device has been on the market and in use since 1979. INC was concerned, however, the safety latch plate bending has never been reported to INC.

INC initiated an investigation and found that whenever the source was in the safe position with the IR 100 locked, the safety latch plate could be bent by a direct blow.

Please return your IR 100 to us foi a modified safety latch plate replacement the next time you need a new so. ve. This will insure that the safety latch plate bending will not occur in the future.

If you can not return your IR 100 to us, please replace the current safety latch plate with the modified safety latch plate we have enclosed with this notice. Note: Source must be properly stored in a Source Chancer before replacing the safety latch pho.

It is expected that recipients will review the information for applicability to their licensed activities, and consider actions, as appropriate, to avolo problems, However, suggestions contained in this information notice are not NRC requirements; therefore, no specific action nor written response is required.

Descriotion of Circumstances The recent retrofit to the safety latch plate (NRC Information Notice 97-35, dated June 18, 1997), which corrected a scenario where the INC Model IR100 radiography camera would not pass the horizontal shock test, as required by 10 CFR 34.20, resulted in the equipment not J

meeting the requirement that the lock should not be easily removed by unauthorized personnel.

- INC and the State of California evaluated the problem, and INC took action to correct the situation,

- The " Safety Latch P: ate" on the IR100 radiography camera has a potentially more " fragile area" (as specified in ANSI N432-1980, Paragraph 8,3.2) than the area previously tested.

Apparently,if the Safety Latch Plate was struck directly on the front surface of the plate (with the source in the safe. locked position), the plate could be deformed and deemed inoperable. It also appeared that the plate might be susceptible to damage if struck from the top and side. To.

ensure that this type of potential damage was not possibie, a bumper hat would prevent 5

IN 97-87 December 12,1997 Page 2 of 2 deformation was welded to the back of the Safety Latch Plate, and the outer top edge of the plate was angled at a 45 to-50-degree angle, to a depth of approximately 3 mm (0.12 inch). The modified device was tested to ensure compliance with ANSI N432-1980, paragraph 8.2.

6 In the Safety Latch plate retrofit to the IR100, the Allen screws used to fasten the plate to the device were such that they were easily removed by the user. This is contrary to requirements in 10 CFR 34.20(a) which states that all devices in use meet the requirements of ANSI N432-1980.

ANSI N432, Section 5.1.2.1, requires that "[t]he exposure device shall have a lock which is not easily removabl9 by unauthorized personnel." To correct this situation, INC has modified the device design to secure the lock screws by placing a self-adhesive metal foil over the screws.

The foil and adhesive are such that the foil is difficult to remove and cannot be removed and replaced without showing signs of damage. The foil will serve to inhibit unauthorized personnel from accessing the screws, and to provide a means to alert the licensee to possible tampering of the lock. INC has indicated that it will contact all owners of potentially affected cameras, on record with it, to notify them of the retrofit, and to retrofit the cameras for them or to provide them with the foil covers and instructions on retrofitting the devices. Attachment 1 contains a copy of the retrofit letter.

All affected cameras should be retrofitted as soon as possible. Licensees should contact INC, or the State of California, to obtain retrofit kits or assistance with retrofit procedures, or to report that they have not received their kits. Affected cameras may be checked for completed retrofit 3 during any inspection.

This information notice requires no specific action nor written response. If you have any questions about the information in this nctice, please contact one of the technical contacts listed below or the appropriate regional office.

The device consists of the following major parts:

A. depleted uranium casting embedding a titanium or zircalloy "S" tube. All contact points between uranium metal and the stainless steel are protected with 0.010" copper.

(2)

A stainless steel box 10-7/8" (length) x 4-1/4" (width) x 9-l/4" (height) and 0.125" thick with a handle heliarc welded on the top.

Source Tube Assembly - A flexible metallic hose consisting of one, two, or three 7-foot sections through which the Source is moved by the Drive Cable.

Control (Crank) Assembly - Consisting of two flexible Drive Cable liousings (25 foot each);

a IIand Crank assembly, provided with a Cable Reel or pistol Grip; and a special swivel connector (with tab) for attaching the Control Assembly to the Device.

The depleted uranium casting with the "S" tube is cased inside the stainless steel box. The space around the casting is filled with polyurethane foam The "S" tube is open to the outside through a hole in each end of the box. A stainless steel fitting is heliarc welded to the outlet end of the hole for attachment of the source tubc(s). A stainless steel lock body is heliarc welded to the opposite hole. The lock body contains a lock cylinder, a latching indicator plate, and a retainer to prevent removal of the source assembly. The retainer fitting is internally threaded to permit attachment of a remote drive assembly. The outlet fitting is provided with a shipping plug assembly which acts as an extra means to secure the source in the safe zone. The latching indicator plate prevents operation of the device in the lock mode, provides a visible indication when the device is in the operable mode and traps the source pigtail assembly in the safe zone. A manual reset of the indicator is required before the source can be moved out of the safe zone.

Failure to connect the Drive Cable to the Source Pigtail Assembly will prevent unlocking of the device. Verification of the connection must be performed (by retracting the Source Pigtail Assembly) before the Device can be unlocked. When the Control (Crank) Assembly is rotated in the " retract" mode, the indicator will " pop-up" to a safety latch position. This function sen*es the purpose of verification that the Drive Cable is connected to the Source Assembly and also indicates, visually, that the Device is unlocked, The safety latch must be depressed before the source can be exposed. When the Source Assembly is fully retracted into the Exposure Device, the latch will engage the Source Assembly. This may be observed by the " pop-up" indicator. Verifier.tica of this condition may be made by rotating the lland Crank in both directions. At this point, the Source Assembly will not crank out until the latch indicator is manually reset (depressed). The Device can be locked only when the Source (Pigtail) Assembly is in the safe position in the Device. The key cannot be removed except when the Device is in the locked condition. The Control (Crank)

Assembly can not be removed until the Device is locked and the key is removed. The special tab on the swivel connector prevents the disconnecting of the Control Assembly unless the key is removed.

The device weighs appmximately 52 pounds. The contained unmium casting weighs approximately 32 pounds.

Compatible source changer with the Exposure Device Model IR 100 is: Industrial Nuclear Model IR 50.
