Subject: NRC Region I Inspection Report No. 50-293/88-12
This refers to the routine safety inspection (50-293/88-12) conducted by Messrs.
C. Warren, J. Lyash and T. Kim of this of fice on March 6 to April 17, 1988 at. *
the Pilgrim Nuclear Power Station, Plymouth, Massachusetts.
Areas examined during this inspection are described in the NRC Region I
Inspection Report which is enclosed with this letter. This inspection noted
continued improvement in the areas of problem investigation and root cause
analysis.
Based on the results of this intpection however, it appears that certain plant
component modification activities were not conducted in full compliance with
NRC requirements, as set forth in the Notice of Violation enclosed herewith as
Appendix A. Portions of a plant design change were not properly reviewed and
released resulting in incorrect equipment installation. .In addition, the
design verification testing did not meet Quality Assurance requirements and
failed to verify the design adequacy. ,
The response directed by this letter and the accompanying Notice are not
subject to the clearance procedures of the Office of Management and Budget as
required by the Paperwork Reduction Act of 1980, PL 96-511.
