APPENDIX A NOTICE OF VIOLATION Houston Lighting & Power Company Docket: 498 South Texas Project, Unit 1 Operating' License: NPF-76 During an NRC inspection conducted on May 2-6, 1988, a violation of NRC requirements was identified. In accordance with the "General Statement of

. Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988),theviolationislistedbelow:

South Texas Project Technical Specification Section 6.8.1.a.' requires that-written procedures shall be established, implemented, and maintained to control maintenance activities.

1 i Station procedures OPGP03-20-0007 (conduct of maintenance) and l OPGP03-ZH-0003 (maintenance work request program) implement the above TS requirements for maintenance activities.

Contrary to the above, troubleshooting by the technician of the steam dump controller, performed on April 14, 1988, using Maintenance Work Request (MWR) MS-55308, failed to control the maintenance activity in that the reactor plant reference temperature (TREF) control signal was lost unexpectedly during work performance. The loss of TREF was caused by Step 5.2.5 of the procedure, which pulled instrument card TY-660A.

Pursuant to the provisions of 10 CFR 2.201, Houston Lightin: & Power Company is hereby required to submit to this office, within 30 days o. the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1)thereasonfortheviolationifadmitted, ,

2) the corrective steps which have been taken and the results achieved,  !
3) the corrective steps which will be taken to avoid further violations, and l
4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time. >
