SUBJECT: PERFORMANCE APPRAISAL TEAM (PAT)/ SAFETY SYSTEM FUNCTIONAL

INSPECTION REPORT 50-289/86-03

This letter fonvards the report of the PAT / Safety System Functional Inspection

performed by an NRC team over the period March 3-27, 1986, involving activities

authorized by NRC Operating License Number DPR-50 for the Three Mile Island - -

Unit 1 facility. This inspection was conducted jointly by members of the

Perfonnance Appraisal Section from the Office of Inspection and Enforcement,

Region I inspectors, and NRC contractors.

At the conclusion of the inspection,

the findings were discussed at an exit meeting with those members of your staff

identified in the appendix to the enclosed inspection report.

The NRC inspection at TMI-1 was conducted in response to Comission Memorandum

and Order CLI-85-09.

In the Memorandum and Order the Commission directed the

staff, in part, to perform a Performance Appraisal Team (PAT) inspection at the

end of six months of operation of THI-1.

As a means of implementing this direc-

tive from the Comission, the inspection team performed an assessment of the

operational readiness and functionality of the TMI-1 emergency feedwater (EFW)

system. Particular attention was directed to the details of modification and

design control, quality of maintenance and testing, and the adequacy of procedural

and training support applicable to the EFW system. Although the primary focus

of this inspection was on the EFW system, the inspection team also assessed the

operational readiness of systems that support the EFW system, such as the

250 Vdc station batteries, the two-hour backup supply air system, and the

integrated control system (ICS).

The report includes findings that may result in enforcement action, which would

be the subject of subsequent correspondence. The report also addresses other

observations and conclusions made by the inspection team.

Section II of the

report is a summary of the more significant findings that affect the operational

readiness of the selected safety systems.

Although many strengths were identified by the team during the course of the

TMI-1 inspection, the primary emphasis of the enclosed inspection report is on

the weaknesses identified.

This emphasis is intended to provide you with a

clearer sense of where additional management attention and involvement may be

appropriate.

In general, the NRC inspection team noted strong performance

at TMI-1 in the areas of training, quality assurance, maintenance, and

The team observed, however, that the performance of activities

related to design changes',and plant modifications did not achieve the high

standards attained in the other areas inspected. Of particular concern was

that the two-hour backup supply air systen that supplies the pneumatically

operated EFW flow control va.lves did not meet the single failure criteria

(observation III. A.l.a.) and the lack of independence between the EFW remote

shutdown panel and control ro~om circuits (observation III.A. 2.b.).

We request

that you respond to this office within 30 days describing the actions you have

taken and intend to take to' improve the overall performance of activities

related to design changes and plant modifications. As part of this response,

please address the corrective actions taken that relate to the two specific

concerns identified above. ,

While planning corrective actions based on the weaknesses identified in the

enclosed report, it is important that you realize that the focus of this

inspection was primarily on the emergency icedwater system at THI-1. Therefore,

consideration should be given to identifying and correcting similar problems in

other safety-related and isportant-to-safety systems.
