(b): February 10, 1986 letter from D. L. Farrar to J. G. Keppler.

Dear Mr. Keppler:

Reference (a) transmitted the Zion SALP Board Report and notified Commonwealth Edison Company of the SALP review meeting, which was held on January 10, 1986. Reference (b) supplied the Company's response to the SALP Board Report.

However, due to a misunderstanding of the scope of the reference (a) request, reference (b) did not address the issue of personnel errors.

Commonwealth Edison Company's response to this issue is contained in the attachment.

If there are any further questions regarding this matter, please direct them to this office.

ATTACEMENT As part of Zion Station's efforts to reduce the number of personnel errors, Zion has taken steps to heighten management awareness of the need to manage these errors. Zion recently adapted the INPO Human Perfor:.ance System (HPES) which is designed to alert management of potential problems so that steps can be taken to prevent personnel errors. The program also includes an investigative function which will augment the present PRO program.

HPES is a voluntary program that encourages station personnel to report potential problems or near-misses to an assigned HPES Coordinator anonymously so that corrective action can be initiated as appropriate.

Events involving personnel error will also be investigated by the HPES Coordinator on a case-by-case basis.

Zion, in addition to the generic programs mentioned above, also responds to each personnel error on a case-by-case basis. The corrective actions listed below have been used to deal with specific events as well as act as a deterrent to future errors.

1. Retraining of personnel involved
2. Diseplinary action
3. Procedure revisions to make more specific
4. Requiring "in hand" procedures on critical tasks
5. Dual verification on some tasks
6. Review of errors and corrective actions with the work group affected
7. Incorporating corrective actions into training programs when appropriate i 1

Zion Station continues to seek ways to prevent errors from occurring but if they do we will continue to investigate the cause and take appropriate l action. l l
