Gentlemen:

This refers to the routine safety inspection conducted by NRC inspectors,

M. M. Holzmer, L. E. Kanter, J. N. Kish, M. McCormick-Barger and A. Dunlop

of this office on November 19, 1985 through January 3, 1986 of activities at

the Zion Nuclear Generating Station authorizeo by License No. DPR-39 and

No. DPR-48 and to the discussion of our findings with Mr. G. Pliml at the

conclusion of-the inspection.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

During this inspection, certain of your activities appeared to be in

violation of NRC requirements, as specified in the enclosed Notice. A

written response is required.

We wish to express our concern regarding the December 14, 1985 event during

which both trains of your residual heat removal (RHR) system were lost. While

the safety significance of this event was considered to be minor, we are

concerned uo'out the effectiveness of your corrective action system considering

the repetitive nature of this event at Zion, and the availability of

information about loss of decay heat removal events which have occurred

nationwide. In addition to your response to the Notice of Violation, please

andress the other concerns highlighted in Paragraph 3 of the enclosed report,

and include any corrective action you plan to take to preclude repetition.

Following the receipt of your response, we desire to meet with you to discuss

this issue.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Rooia.
