APPENDIX A NOTICE OF VIOLATION TU Electric Dockets:

50-445/88-54 50-446/88-50 Comancho Peak Steam Electric Station Permits:

CPPR-126 Units 1 and 2, Glen Rose, Texas CPPR-127 During an NRC inspection conducted on August 3 through September 8, 1988, a violation of NRC requirements was identified.

In accordance with the "Goneral Statement of Police and Proceduro for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is lisced below:

Special Nuclear Material License 1912 dated February 14, 1983, governs the storage and possession of nuclear fuel.

Part VII of the license, entitled "Radiation Safety," states that the radiation protection engincor is responsible for implementation of ti:o health physics program and that CPSES is committed to r,stablishing a program to maintain occupational and general pr.hlic exposure to radiation As Low As Is Reasonably Achievable (MLARA).

The administrative controls used are covered by hoolth physics procedures.

These proceduros include the issvance and use of radiation permits.

CPSES Station Administration Manual, Procedure STA-656, "R0diation Work control," states that all routine encries into a radiattoa controlled area require a general access permit or a radiation work permit.

Contrary to the above, on August 14 and 15, 1988, a contract employee working in the fuel building crossed a barrior into a radiation controlled area without meeting the required administrative controls.

This is a Severity Lovel IV violation (Supplement IV)

(445/8854-V-01).

Pursuant to the provisions of 10 CFR 2.201, TU Electric is hereby required to submit a written statement or explanation to the U. S.

Nuclear Regulatory Coinmission, ATTN Document Control Desk, j

Washington, DC, 20555, with a copy to the Assistant Director for Inspection Programs, Comanche Peak Project Division, Office of Special Projects, within 30 days of the date of the letter transmitting this Not!.co.

(1) the reason for the violation if admitted, (2) the corrective stops that have boon taken and the results achieved, (3) the corrective stops that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If ar. adoquate reply is not received within the timo specified in this Notico, an order may be issued to show causo why the license should not be modified, suspended, or revoked or why such other action as may be propor should not be taken.

Where good cause is shown, consideration will be given to extending the response time.
