Dear Sir On March 10, 1988 during a telephone conversation between Yankee Atomic Electric Company (YAEC), Mr. M. Fairtile (USNRC), Mr. G. Lipinski (USNRC), and Mr. R. J. Eckenrode (USNRC), it was requesten that Yankee provide additional information to enable the staff to verify that our panel improvement provides a satisfactory resolution to the Human Engineering Deficiencies (HED) as previously addressed in Appendix B and Appendix D cf Refort.nce (b).

As requested, attached is a copy of the updated Control Room Design Review (CRDR) data management sort (Attachment A) that provides (1) a cross-ref erence between the old HED numbers and individual findings, (2) reference notes, and (3) current status. The reference notes (Attachment B) provide detailed information of what corrective actions were taken to resolve the HED. The Main Control Room CRDR panel number designations are defined in Attachment C, and plant Drawing No. 9699-FE-23A provides additional details of the Control Room arrangement. As a result of the corrective actions implemented, a new series of half-scale panel front-view drawings were generated, examples of these drawings are provided and listed as Attachment D. To convey a more realistic feel of the corrective j actions taken, example photographs are provided as Attachment E.

As can be seen from our HED listings, drawings, and photographs, a great l deal of work has been accomplished in the Control Room. Yankee has corrected j more than 607. of the findings. Our schedule is to complete the remaining HED j findings over the next two-to-three refuelings. None of the outstanding l findings are major and they are being integrated into an engineering and I modificat. ion schedule.

Note Description 28 The finding noted that the controls for the Emergency Atmospheric Steam Dump (EASD) valves are mounted on tne vertical panel 4" beyond the anthropometric guidelines.

The finding is partially correct. The EASDs were previously discussed under CRDR Panel 9. RCD Key 0438, HED 1422. The controls were mounted on the vertical section, but were relocated under EDCR 83-31, "Thermocouple Upgrade for ICC System." The controls are mounted on the desk section approximately 29" from the edge of the board, which is approximately 0.7" beyond (takes into account shoulder height of fifth percentile female) the extended functional reach of the fifth percentile female.

No action required. '

29 The finding noted that when testing the diesel generator, the operator must check MCB for load indication.

Plant Procedure OP-4207, "Surveillance of the Station Power System and the Emergency Diesel Generators " is applicable. Step 2.k directs the operator to verify that the 480 V bus amps drops; it does not provide information as to where to read this (MCB Panel 13F).

The necessity to go from the EDG panel to the MCB is an inconvenience, but duplicating MCB bus amps indications on the EDG is not possible nor practical due to space limitations. In addition, an exact reading of bus amps on MCB Panel 13F meter is not necessary.

The procedure requires that the operator note a decrease in amps only.

Plant Operations Department has revised Procedure OP-4207, Step 2.k by adding the location of meters to be read (i.e., MCB Panel 13F).

Note Description 30 The finding noted that communication with the MCB is required when operating TV-411.

'IV-411 is an atmospheric steam dump, upstream from the NRVs, used by the Reactor Engineering Department during low power physics testing.

The Reactor Engineers set up a table with portable test gear between the NI panels and MCB. Undur these conditions, the placement of TV-411 is justified. It is also noted that TV-411 could be used by the MCR operators to control MC temperature, but the use of TV-411 in this manner is not called out by any plant procedures.

No action required.

31 The finding noted that placemerit of Battery No. 3 ground voltmeter is inconsistent with others.

The finding is correct. In addition, it is felt that the 2400 V voltmeter and 480 voltmeter should be moved to enhance the display / control relationship.

Also, on CRDR Panel No. 10 (MCB Panel 9F), the display meters for circulating water pumps, condensete pumps, and heater drain pumps should be moved to enhance the display / control relationship. The moving of indicators is reconsnended over moving of controls to limit the possibility of inadvertent operation.

1. Nuclear instrunent power range meters have minor divisions of 2.5 (percent power). The NI power range meters will be changed to digital meters (see Note 70) with a 3% bargraph and numerical readout to tenths of percent.
2. MC pressure meters (MC-P-100, 200, and 300) have 50 psi minor divisions, procedure calls fo.' maintaining pressure between 820 psi co 900 psi. OP-3100 EOP No. 6. Alternate Step 2.5 has teen changed to read 800 psi to 900 psi.

The Human Factors guideline Specification YRS-013 states that lamp test features are required for annunciator windows only.

Incorporation of a lamp test feature may violate safety train separation, increase risk of single point failures, and require complex and costly logic circuitry (that would be difficult to locate in existing panels). In addition, some Type W switches have a target window that would indicate what light should be lit. Also, operators would be able to determine if a control switch has perf ormed its intended action by monitoring metera for chs ge in 16- 1, flow pressures, ete.

The test switches are a key-locked-type by design (to prevent inadvertent valvo closure) whereas the trip switches are not. The size difference was caused by vendor equipment availability.

48 The finding noted that markings between emergency and nonemergency switches are not clear.

The four switches that the original CRDR Team referred to as test switches are actually two trip switches and two block switches. The trip switch handles will be color-coded red while the block switch handles will be color-coded yellow.

Switch handles will be color codes as stated, finding recategorized to color. See Note 1 also.

49 The finding noted that Sigma indicators use setpoint lights differently.

Sigma meters for SF-LIS-225A, and 224A (on the primary pumps control panel) have setpoint lights on if pointer is between setpoints.

Sigma meters for DW-LI-230 (on gravity drain transfer panel) have setpoint lights off if pointer lo between setpoints. General operator preferences are to have setpoints off and then light if pointer goes outside the setpoint limits. Meter is Sigma 9262X zero-powered indicating alarm / controller.

The Sigma meters can be modified by moving the lights lead wire from the clarm points to the control points on the printed circuit boards. I Action - Modify meters for SF-LIS-225A, end 224A so that the setpoint I

The new wide-case model provides approximately two hours of treud information versus the half hour available with the narrow-case model.

52 The finding noted that the steam generator low level trip alarm has eight input signals, but any one of the eight signals can block out I

the other seven. The finding further stated that the E0Ps require a 2/4 low level alarm, but this is not present.

The steam generater narrow-range level alarm and scram channels (one for each steem generator) consist of a transmitter, power supply, scram bistable, indicator / alarm unit, indicators / normal / test switch, and test device. The indicator / alarm unit has hi dh and low level relay contacts for each cteam generator (total eight signals), these signals are tied in parallel to Annunciator N-C, Windo4 11. If this ,

alarm annunciates, the operator must view the steam generetor level indicator to determine which steam generator alarmed and wLether it was a high or low level alarm.

Purification cooling flow lineup is an auxiliary operator task. LPST has a level indicator; flow is determined from level change. LPST make-up line is isolated by manually operated valves (PAB). Shutdown cooling flow is indicated on the shutdown valve panel (CaDR Panel No. 06).

59 The finding noted that the scram push button and trip is not protected.

The trip button is presumed to refer to the condensate pump trip switch that is mounted about 28" f rota the edge of the bench. The scram push button is located aboat 27" from the edge of the bench.

Scram push buttons will not be covered since that would increase operator time to scram the plant. Becausa of their location, inadvertent operation of these controls is unlikely, and they are within anthropometric guidelines.

60 The finding noted that SI-MOV-22 through SI-MOV-25 do rot have position indication.

Position indication is not present because the breakers for these valves are racked-out (EMCC2), the valves are left open, and randomly checked as part of routine surveillance. These MOVs are left open as a result of concerns from single failure, i.e., fire, spurious operation of valves in a detrimental manner. This change took place as part of Proposed Change No.160 (PC-160-1, WYR 78-48) submitted on June 6, 1978. MOV-22 to MOV-25 are the LPSI header isolation B-29 6717R/20.649

62 The finding noted that the SI relay reset switches require greater than nor al force to reset.

The switch in question in a Westinghouse Type W lock-out relay commonly used throud h out the industry. It is electrically activated when the Safety Injection System trips and is manually reset by operators. The frequency of use is very low, and testing is performed during refueling outages.

NUREG-0700 does not provide specific toraue values for "J" handles; however, it does note high-torque J handle dimensions of which the SI relays fall under. M11-Std-1472 does not discuss J handle controls.

The original CRDR basis for this finding was the static survey; however, original team membecs contacted do not recall if the switch was actually testec and a torque value obtained, nor did they have any value to compare to.

Note Descripticn 63 Finding noted that additional indication for steam dump position and digital indication for MC pressure level is needed.

This finding was based on operator comments. The operator . survey summary was reviewed for additional details. The survey noted that operators wanted steam dump position indication for the condenser -- this has been added by EDCR 83-3 during the 1984 refueling outage. The survey niso noted that the operators wanted a larger scale for MC pressure at low operating pressures of 0 psi te 500 psi and a meter indication of pressurizer narrow-range level (at present only on strip chart). The CRDR Team decided a digital meter would provide the resolution that the operator derived.

Increased resolution of MC gressure is neede, for LTOP cperation when starting up from cold shutdown. The oper'.urs transition from low to high when T = 380 F and P = 450 psi, be.; arast maintain pressure (prior to this transition) between 600 psi and 450 psi per OP-2100.

Plant personnel have expressed on additional concern of accuracy, i.e., should a new 0 psi through 500 psi transmitter (to be valved in/out as needed) loop be added rather than adding a digital indicator to the exi. ting MC-PT-712 loop. The existing ar.alog meter has an error (root-sum-square method) of 2.5% cf span, whereas a digital meter (e.g., LFE Medel 50) has an error of 0.01% of reading.

The 'ollowing action will be taken:

1. Add a digital indicator to the MC-PT-712 loop for MC pressure indication and locate meter near the LTOP controls.
2. Add an analog indicator for pressurizer narrow-range level.

Exact locations will be coordinated with the plant Operations Departments.
