Inspection Report No. 50-29/88-02

This refers to the Safety Assessment Team inspection conducted by Mr. N.

Blumberg of this office on January 10-15, 1988 at Yankee Nuclear Power Station,

Rowe, Massachusetts and Yankee Atomic Electric Company, Framingham Massachusetts

of activities authorized by NRC License No. 50-29 and to the discussions of our

findings held by Mr. N. Blumberg with Mr. N. St. Laurent and other members of

your staff at the conclusion of the inspection.

Areas examined during this inspection are described in the NRC Region I

Inspection Report which is enclosed with this letter. Within these areas, the

inspection consisted of selective examinations of procedures and representative

records, interviews with personnel, and observations by the inspector.

Based on the results of this inspection. ". appears that one of your

activities were not conducted in full compliance with NRC requirements, as set

forth in the Notice of Violation, enclosed herewith as Appendix A.

This vio-

lation has been categorized by severity level in accordance with the "General

Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2,

Appendix C (Enforcement Policy).

You are required to respond to this letter.

and in preparing your response, you should follow the instructions in

Appendix A.

Although the attached violation addresses the use of commercial grade fuses in

a safety related system, your response should address your process to ensure

that any commercial grade items are properly qualified for use in safety

related systems. A second part of the violation noted that fuse sizes were not

specified in drawings for the system indicating a potential configuration

control problem.

This inspection determined that configuration control for

recent design changes is acceptable. However, in your response you should

address your mechanisms for configuration control upgrade for design changes

completed before improvements in your quality assurance and design cnange

processes were implemented.

In addition to the above violation, there is an unresolved item concerning

~

problems with some of your main steam line pressure switches which are detailed

in paragraph 7 of the attached report. Although, corrective actions have been

underway for almost.three years, the problems have not been corrected. It

appears that your corrective actions may'have.been slow in being accomplished.

l

Accordingly, we request that you respond to the following areas of HRC'

concerns:

(1) Actions taken to preclude exceeding the~ Technical Specification

limiting conditions for operations for future operations.

(2) Actions taken to ensure that the pressure switch problems are

corrected.

(3) Your assessment of the apparent lack of promptness in correcting this

,

problem.

(4) Actions taken as planned to determine the reportability.of the

pressure switch failures.

The purpose of this inspection was to evaluate overall strengths and weaknesses

in your plant operations.

These are detailed in the enclosed inspection

report; however, certain key strengths and weakness are summarized below.

In

general, the inspection team found the plant to be safely operated, well

maintained, and adequately staffed.

The following strengths are noted (others

are identified in the report):

There is a stable plant staff which is committed to safe

conservative operations.

Control room operations were excellent and operators displayed

-

positive attitudes.

-

Maintenance craftsmen exhibited a pride in performance and a

knowledge of maintenance.

-

Plant equipment was well maintained.

There was excellent corporate engineering support for design

-

modifications.

The following weaknesses are also noted:

There appeared to be an occasional lack of aggressiveness in

resolving problems observed during plant operations and a lack of a

questioning attitude,

The Maintenance Support Department concentrates on modifications

-

rather than the plant maintenance activities.
