PROCUREMENT AND MATERIALS CONTROL INSPECTION (50-312/88-02)

This letter transmits the report of the inspection conducted January 4 through

14, February 10 through 12, and March 7 through 9,1988, at the Rancho Seco

Nuclear Generating Station by Messrs. R. P. McIntyre and J. B. Jacobson of the

Vendor Inspection Branch and Mr. W. J. Wagner of Region V.

The inspection was

related to activities authorized by NRC License Number DPR-54.

At the conlusion

of the inspection, the findings were discussed with Mr. Gary J. Legner and other

members of your staff.

The purpose of the inspection was to review the implementation of your program

for procuring items for safety-related applications and your material control

j

program, both past and present. The results of the inspection identified

i

weaknesses in the procurement program; specifically, the coninercial grade item

dedication process ano the auditing and placement of vendors on the approved

suppliers list (ASL).

The inspection findings identified include: (1) failure

to properly dedicate and provide documentation for several comercial grace

items used in safety-related applications (these items were accepted on the

basis of a "like-for-like" comparison of part numbers without the licensee

adequately) evaluating their suitability for use in these safety-related applica-

tions); (2 failure to impose the requirements of 10 CFR Part 21 on vendors

where certifications to nuclear specifications were required; and (3) failure

to adequately evaluate vendor quality assurance (QA) programs before placing

the vendors on the approved supplier list (ASL).

Also, certificates of

conformance (C of Cs) for comercial grade procurements from vendors not on

the ASL, were accepted without verification of the validity of the C of Cs.

Certain findings discussed in the report have been ciassified as potential

enforcement findings (50-312/88-02-01. -02 and -03 encloseo) and will be

referred to the NRC Region V office for appropriate action,

POTENTIAL ENFORCEMENT FINDINGS

As a result of the inspections conducted from January 4 through 14,' February 10

through 12, ano March 7 through 9,1988, and in accordance with the "General

Statement of Policy and Procedures for NLC Enforcement Actions " 10 CFR Part 2. Appendix C (1986), the following potential enforcement findings were

identified:

A.

The NRC inspectors found that several purchases of spare and replacement

l

comercial grade items to be used in safety-related applications were

accepted on the basis of a "like-for-like" compariscn of part numoer

l

l

without adequate evaluation of their suitability for use in safety-related

l

applications.

Specifically:

1.

A COMSIP catalyst assembly for the hydrogen monitor was purchased

without qualification documentation for use in an environmentally

qualified (EQ) application. Also, no certificate of Confontance

(C of C) for this item was received.

2.

An Agastat relay installed in the undervoltage protection sys.em was

received without any seismic qualification or verification that no

design cr manufacturing changes had been made.

3.

A K3 connercial grace relay was received without any seismic qualifi-

cation or verification that no design or n.anufacturing changes had

been made.

4.

A circuit breaker for use in the "A" inverter vital bus was

functionally tested upon receipt without acceptance criteria being

established (i.e., the trip times were unkncwn.) Also, no verification

of cesign or manufacturing changes was performed.

A General Electric (GE) relay in the reactor sampling system was

accepted without seismic qualification or verification that no

design or manufacturing changes had been trade.

A Wilnar relay in the reactor coolant pump underpcwer protection

cabinet in the reactor coolant system was accepted without seismic

Conn.ercially procurec Ohmite relays for the B norr.al battery charger

were received with a different configuration and accepted on the basis

the validity of the C of C.

B.

Po'+ntial Enforcement Finding 50-312/88-02-02

The NRC inspectors found two instances of procurement of safety-related

items that did not include a requirement for the vendor to meet 10 CFR Part 21.

1.

The purchase of the five thousand feet of 5 kV power cable from NSSS/

DIVESCO, Inc., did not impose the requirements of 10 CFR Part 21;

however, generic test data performed to IEEE standards 383 and 323/324

was requested. This cable does not qualify as a commercial grade

item as defined by 10 CFR Part 21.

2.

Commercial grade replacement parts for Bnrgen-Patterson hydraulic

snubbers were procured during 1980 through 1985 and required a C of C

to nuclear specification (ANSI B31.7, "Nuclear Power Piping").

SMUC

never imposed the requirements of 10 CFR Part 21 on Bergen-Patterson.

It should also be noted that during the SMUD evaluation of their Class 1

items with green tags,11 of 75 purchase orders failed to impose 10 CFR Part 21 reporting requirements on the vendors as required.

C.

Potential Enforcement Finding 50-312/88-02-03

Safety-related procurements were made from vendors placed on the SMUD

approvcd supplier list (ASL) solely on the basis of desk audits of the

quality assurance (QA) manual.

Even after SMUD upgraded their process

for approving suppliers in late 1987, SMUD procedures still albwed

procurements to be made with no established time limits for performing

some form of proaram implementation evaluation.

This process could

potentially allow items to be procured and placed in service without the

vendor ever rec 61ving a QA program implementation evaluation.

Also, C of Cs for comercial grade procurements from vendors not on the

ASL were accepted without verification of their validity.
