HIGHLlGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING AUGUST 12, 1988 SEABROOK UNIT I Public Service Company of New Hampshire (PSNH) recently filed a notice of intention with the U.S. Bankruptcy Court proposing the spin-off of New Ham) shire Yankee Division (NHY) into an inuependent company, the New Hampshire Yan(eeElectricCorperation(NHYEC).

When NHY was first fonned in 1984, it was the intention of the joint owners to eventually establish NHYEC and to have PSNH transfer to NHYEC the Seabrook construction and operating license. The proposed restructuring is not to affect ownership of Seabrook, but it would transfer management authority to NHYEC.

The Joint Owners have apparently received necessary approvals from all the public utility commissions in New England with the exception of the Massachusetts' Department of Public Utilities. As yet, the Joint Owners have not sought approval from the NRC.

Peach Bottom The licensee's monthly schedule update dated August 3, 1988 states that 89 percent (123 of 138) of the activities in the Restart Plan are complete.

The licensee states that the extension of those items not yet completed is not expected to change the licensee's projected restart schedule. The licensee's schedule includes an INP0 restart readiness inspection in mid-September and readiness of Unit 2 for startup in November, 1988.

Comanche Peak Members of Citizens for Fair Utility Regulation (CFUR) held a news conference Thursday, August 11, in front of the building where Region IV offices are located to announce its filing of a petition seeking readmission as a party in the Comanche Peak licensing proceeding. CFUR, once an active intervenor withdrew in 1982 in favor of Citizens Association for Sound Energy (CASE}. At the news conference, the organization gave reporters copies of its basic petition which it filed by mail with NRC that day. The Fort Worth (Texas)

Sierra Club also distributed copies of a letter supporting CFUR's action.

Three newspaper reporters and one radio reporter attended. The Region IV public affairs officer answered two questions from reporters about procedural matters associated with intervention petitions.

CFUR and the Sierra Club both oppose the recent settlement between CASE and TU Electric which has been agreed to by the NRC staff and accepted by the presiding Atomic Safety and Licensing Board.

Missing 3M Shipment Region III notified IP August 8 that a July 28 shipment of four radioisotopes of one mil 11 curie each, from the 3M Company to the Victoria Hospital, London, Ontario, Canada, is missing.

The package was picked up by Burlington Northern Air Freight but never arrived at its destination. The 3M Company has re-shipped the order and is attempting to trace the original shipment.

Minnesota state authorities have been advised of the missing shipment.

Week Ending August 12, 1988 Arkansas Nuclear One, Unit 2 (ANO-2)

ANO-2 remains in Mode 5 while design changes are being developed and implemented to preclude future leaks or L eaks of reactor coolant pump (RCP) seal cavity pressure sensing lines.

The break on August 1, 1988 in the 'A' RCP middle seal pressure 3/8 inch sensing line resulted in an approximately 40 gpm maximum leak rate and damage to the lower, middle, and upper RCP seals; about 10 gpm of the leakage was passing by the vapor seal at the top of the seal package. Total leakage was estimated to be approximately 15,000 gallons. The 3/8 inch instru-ment line break occurred just downstream of a socket weld in the 3/4 inch pipe to 3/8 inch instrument line transition and is believed to have been due to i

fatigue failure. The failed line and two other cavity pressure s osing lines with similar configurations are being modified. Additionally all tther connec-tions to the pumps (controlled bleedoff, the remaining pressure sensing lines, and the low-pressure-use seal injection lines) are being evaluated to verify design adequacy. ANO-2 has Byron-Jackson PCPs, but the arrangement of the seal l

cavity pressure sensing lines is thought to be plant specific and not generic.

A conference call between the staff, Region IV, and the licensee is planned for 11:00 a.m. (EST) on August 9 to discuss corrective actions and plans for heatup i

and startup.

Bulletin 88-05 Supplement 2 to Bulletin 88-05 was issued on August 3, 1988.

The second supplement to Bulletin 88-05 temporarily suspends the field measure-ments, testing, records review and the preparation of justifications for continued operations (JCOs) that were requested by Bulletin 88-05 and l

supplement until further notice.

NT0Ls are requested to continue the in-situ testing and the records review.

Indian Point 2 The Indian Point 2 (IP-2) Updated Final Safety Analysis Report and Technical Specifications (TS) establish a maximum service water temperature limit of 85*F to ensure adequate cooling capability for essential loads (e.g., emergency diesel generators, containment fan cooler units and component cooling water).

The service water supply, and thus the ultimate heat sink for IP-2, is the Hudson River.

1 Due to combination of the unusually hot summer, the nation's drought, and tidal action in the Hudson River causing the IP-2 service water system to intake water from the Indian Point 3 service water discharge plume, the service water temperature at IP-2 peaked several times above 85'F on August 3, 4 and 5,1988.

j Each time the service water temperature exceeded 85'F, the licensee comenced shutting down as required by TS LC0 3.0.1 which states that the plant must be j

J On August 4, 1988 the licensee submitted an emergency request te delete the limit on service water temperature. The NRC staff determined that the licensee had not presented an adequate technical basis to support this request. Subsequently, on August 5, 1988 the licensee supplemented and modified this eme gency amendment request by supplying additional technical justification and by proposing that the maximum service water temperature limit be raised to 87'F rather than having it deleted. The NRC staff found the additional justification sufficient to support a temporary change to this temperature limit.

Consequently, at 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br /> on August 5, 1988, a waiver of compliance with the 85'F service water temperature limit was granted by the NRC to provide adequate time for processing the emergency amendment request.

This waiver permits the licensee to operate with a service water temperature of 87'F or less.

This waiver did not become effective until the licensee completed 1) the development of emergency operating procedures to maintain component cooling water heat exchanger outlet temperature at 150*F or less and 2) training the plant operators upon these emergency operating procedures.

j Subsequent to the issuance of this waiver, the licensee informed'the NRC staff on August 8,1988 that service water temperature exceeded 87'F at least twice over the period of August 5-7, 1988. Consequently, the licensee stated that j

they intend to change their emergency amendment request to permit power operations at service water temperatures of 90*F or less. The licensee intends to submit this additional modification, including the applicable technical justification, j

by the close of business on August 11, 1988.

0FFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending August 12, 1988 LOW-LEVEL WASTE MANAGEMENT Meeting With Japanese to Discuss Waste Classification On August 8,1988, staff representatives met with Japanese Utility Representa-tives to discuss waste classification, below-regulatory-concern wastes and waste solidification. Japanese representatives indicated that Co-60 levels in j

wastes from their new plants were very low because of substitution of stellite i

materials and specification low robalt levels in stainless steel. At these plants occupational exposures to all personnel totalled about 20 man-rem /yr.

l INDUSTRIAL AND MEDICAL NUCLEAR SAFETY Fuel Cycle Safety Babcock & Wilcox Naval Nuclear Fuel Plant (B&W)

A member of the Fuel Cycle Safety Branch participated in a Region II meeting with B&W management and staff to discuss actions to be taken by B&W prior to startup of unclad uranium process operations in the Naval Nuclear Fuel Plant.

B&W voluntarily shut down these operations during an NRC inspection (July 18-22, 1988) that revealed deficiencies in the licensee's nuclear criticality safety controls. As stated in a Confirmatory Action Letter issued by Region II, the licensee agreed that affected operations would not be restarted without NRC approval.

The B&W action plan described at the meeting covered short-term and long-term corrective actions. Region II, with assistance from NMSS, is considering the B&W actions and any conditions deemed necessary for startup, as well as possible enforcement actions.

SAFEGUARDS In_ternational Domestic Category I NRC/ DOE Comparability Upgrades Guidance for the implementation of the NRC/ DOE comparebility measures is being prepared for publication and public coment. This guidance, in the fonn of three separate NUREG's, concerns vehicle and protected area barriers, package search, and exterior intrusion detection system.

Information Notice The staff has completed an Information Notice to all holders of NRC Quality Assurance (QA) Program Approvals for Radioactive Material Packages. 'The notice informs addressees of problems found in recent inspections, and request that they review the implementation of their QA programs and take corrective action as appropriate.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending August 12, 1988 ACRS Subcommittee Review on Chernobyl Follow-up RES, supported in appropriate areas by NRR staff members, met with the ACRS Subcommittee on Safety Philosophy, Technology, and Criteria on August 4, 1988, for a review of Chernobyl follow-up status and plans. The review included a general overview cf the status of the Chernobyl Implications Report (NUREG-1251), including disposition of public comments on it, and of the status and plans concerning the follow-up research resulting from the NUREG-1251 recommendations. More detailed reports on the follow-up research plans in the areas of human factors, containment, and emergency planning were included, as was a detailed progress review on the Reactivity Transients task. The latter included a presentation by Brookhaven National Laboratory on approach, results to date, and plans for the remaining work under BNL's NRC contract on this task. The discussion indicated the Subcommittee to be generally satisfied with the directions being taken. Views and suggestions expressed in the discussion will be taken into account in the further conduct of the work.

Items of Interest Office for Analysis and Evaluation of Operational Data i

Week Ending August 12, 1988 Diagnoftic Evaluation and Incident Investigation Branch (DEIIB)

The Femi Diagnostic Evaluation Team held its first team meeting on August 8 and 9.

Presentations were made to the team by both NRR and Region III personnel in addition to those by AE0D. The team has now begun to develop specific evaluation plans for each functional area to be evaluated.

Incident Response Branch (IRB)

On August 9-10, IRB conducted the first of a series of training courses for the regional offices at Region III. The course covered the protective measures technical procedures and computer tools.

On August 9, the AEOD Director attended the first meeting of the COSMOS 1900 (Soviet Nuclear Satellite) Food and Health Effects Advisory Group. The participants discussed the organization of the group, approved the charter arid j

objectives and will meet periodically until COSMOS 1900 reenters at which time it will be determined what additional actions need to be taken.

On August 9, the IRB Chief made a presentation on the NRC Incident Response Program at a Mid West Nuclear Security Association Seminar attended by utility and law enforcement representatives.

On August 12, IRB conducted a tour of the Operations Center and briefing of the Incident Response Program for senior utility managers from the Institute of Nuclear Power Operations.

Onthrio, Canada, is missing. The package was picked up by Burlington Northern Air Freight but never arrived at its destination. The 3M Company hcs re-shipped the order and is attempting to trace the original shipment.

Minnesota state authorities have been advised of the missing shipment.

Foreign Visiters On Thursday, August 11. Masanobu Ito, Vice General Secretary of the Hokaido Prefectural Headquarters of the Socialist Party of Japan, met with staff from NRR, NMSS and GPA/IP to discuss reactor safety and spent fuel safety issues.

STATE, LOCAL AND INDIAN TRIBE PROGRAMS Pilgrim Integrated Assessment Team Inspection Two representatives from the Commonwealth of Massachusetts are participating as observers of the NRC Integrated Assessment Team Inspection of restart readiness for Pilgrim. The IATI began on August 8, 1988 and is scheduled to last about two weeks. The exit meeting is tentatively scheduled for August 24, 1988.

Vermont Low-Level Radioactive Waste Meeting Don Nussbaumer, Assistant Director for State Agreements, SLITP, and Marie Miller, Region I State Liaison Officer, participated in a joint meeting of the Vemont Advisory Comission on Low-level Radioactive Waste and the House Natural Resources and Energy Committee on August 11, 1988 in Montpelier, i

Vermont. Mr. Nussbaumer described the NRC Agreement State Program and responded to questions on the technical, staffing and administrative considerations regarding Agreement State status, including regulation of a low-level waste disposal facility.

Charles Hackney, Region IV State Liaison Officer, met with Auburn Mitchell, the Texas State Liaison Officer, and other State officials on August 9, in Auttin, Texas. On August 10, Mr. Hackney met with Dale McHard, the Oklahoma State i

Liaison Officer, and other Oklahoma State officials in Oklahoma City, Oklahoma.

The purpose of these meetings was for Mr. Hackney, the newly-appointed RSLO in Region IV, to meet State officials and discuss areas of immediate concern. The discussions focused on low-level waste, emergency planning and NRC training for State personnel.

l Office of Enforcement Items of Interest Week Ending August 12, 1988 The following Significant Enforcement Action was taken the week of August 8, 1988.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $1,250,G'O was issued to Philadelphia Electric Company (Peach Bottom Units 2 and 3) on August 10, 1988. Additionally, Notices of Violation and Proposed Imposition of Civil Penalty were issued August 9, 1988 to various licensed and former licensed operators. The actions were based on widespread inatten-tiveness in the control room that resulted in the March 31, 1987 Shutdown Order.

Region IV Week Ending August 15, 1988 1.

Comanche Peak A management meeting was held in the Region IV office on August 8 with TU Electric corporate officers to discuss the performance of CPSES applicants for Senior Reactor Operator and Reactor Operator Licenses on NRC administered exams. This meeting was being held in accordance with the TV Electric / CASE settlement agreement.

C_o_manche Peak Members of Citizens for Fair Utility Regulation (CFUR) held a news conference Thursday, August 11, in front of the building where Region IV offices are located to announce its filing of a petition seeking readmission as a party in the Comanche Peak licensing proceeding. CFUR, once an active intervenor, withdrew in 1982 in favor of Citizens Association for Sound Energy (CASE). At the news conference, the organization gave reporters copies of its basic petition which it filed by mail with NRC that day. The Fort Worth (Texas)

Sierra Club also distributed copies of a letter supporting CFUR's action.

Three newspaper reporters and one radio reporter attended. The R6gion IV public affairs officer answered two questions from reporters about procedural matters associated with intervention petitions. CFUR and the Sierra Club both oppose the recent settlement between CASE and TV Electric whi d has been agreed to by the NRC stiff and accepted by the presiding Atomic Safety and Licensing Board.

South Texas At approximately 10:56 a.m. (CDT) on Saturday, August 6,1988, with South Texas Project, Unit 1 (STP-1), at 100 percent power, the No. 12 main feed pump (MFP) was tripped because a MFP controller fuse failed. Reactor power was reduced to below 87 percent to repair the controller. The MFP trip occurred when STP-1 was approximately 71 hours8.217593e-4 days <br />0.0197 hours <br />1.173942e-4 weeks <br />2.70155e-5 months <br /> into a 100-hour NSSS Acceptance Test. The reduction in power invalidated the test. At approximately 4:30 p.m. (CDT) on Sunday, August 7, 1988, STP-1 began a return to 100 percent power. Leakage past the "C" main feed water regulation valve packing may cause a further delay in recommencing the NSSS Acceptance Test.

Meetings with State Liaison Officers On August 9, 1988, the RSLO met with the Texas State Liaison Officer, representatives from both the Bureau of Radiation Control and the Texas Low-Level Radioactive Waste Disposal Authority.

On August 10, 1988, the RSLO met with the Oklahoma State Liaison Officer, and representatives from the Environmental Health Services.

The purpose of the meetings is to meet with state representatives and discuss any areas of imediate concern.

On August 9 Tropical Storm Warning was in effect along portic.cs of the Gulf Coast, including the area of Waterford 3 Steam Electric Station. The licensee postponed surveillance and preventive maintenance activities that could adversely affect plant reliability, such as scheduled preventive maintenance on an emergency diesel generator that was to begin on August 8, 1988. As of August 9. Waterford 3 had not been affected by the storm conditions, although there was considerable local flooding and high winds in the New Orleans area. The tropical storm is expected to continue drifting east and not make landfall on the Louisiana coast.

Arkansas Nuclear One - Unit 2 Region IV and NRR representatives were briefed via a conference call of August 9, 1988, by Arkansas Power and Light representatives on the cause of the sensing line failure of Reactor Coolant Pump A, which was discussed in a preliminary notification (PNO-88-62) issued August 2, 1988. The licensee also discussed proposed modifications and post-modification testing required by the engineering organization prior to restart, which is not expected before the end of the week.

The licensee agreed to brief Region IV cnd NRR representatives before restarting the plant. The NRC staff provided positive feedback with respect to the level of engineering evaluation being conducted in response to the sensing line failure.

Penn Inspection Company An enforcement conference was being held on August 11 in Region IV offices witn Penn Inspection Company, Chickasha, Oklahoma. The conference is a result of a potential overexposure of a radiographer and a t;bsequent NRC inspection conducted on May 31, 1988. On May 12, 1988, the licensee's radiographer had difficulty in returning a 45-Ci iridium-192 source to a safe position after performing an exposure, and straightened the guide tube with his hands. He was not wearing any personnel dosimetry at the time. He subsequently became concerned about his exposure and sought medical attention. The doctor notified the state of Oklahoma, which then notified NRC. The NRC Inspection established that a significant exposure had not occurred, but identified seven apparent violations.

Cooper Nuclear Station D. E. Schaufelberger, President and Chief Executive Officer of Nebraska Public Power District announced that he will retire in early 1989.

Fort Calhoun Station An enforcement conference was held with Omaha Public Power District and Region IV management on August 11, 1988, to discuss matters concernir.g fuel cycle 11 setpoint errors.

10, Fort Calhoun Station Omaha Public Power District reported on August 9, 1988, that they had recently changed contractors that provide health physics technicians.

During the routine screening and evaluation program administered to 28 new health physics technicians, only 6 successfully passed the written 4

examination.

It was reported that the 22 individuals that did not pass the examination planned to obtain employment at facilities outside of Region IV. Headquarters and the other affected regions have been notified.

The order held that the NRC retains jurisdiction over the Kress Creek materials and that the Commission has relinquished jurisdiction over the 1

EE.QY-88-184 - LICENSING BOARD DECISION ON SENIOR REACTOR OPERATOR LICENSE FOR DAVID W. i!ELQ The commission by a 4-0 vote approved an order which completed its consideration of January 11 and February 2, 1988 decisions of the Administrative Judge presiding over the request of Mr. David W. Held for a hearing on the denial of a senior reactor operator license.

The order remands the case to the Administrative Judge for a proceeding on the specific issue of whether Mr. Held should have been found to have passed or failed the simulator examination.

Chairman Zech had additional views which were attached to the order.
