ATTALHMENT I
Violation:
10 CFR 20.201(b) requires that each licensee make such surveys as may be
necessary to comply with all sections of Part 20. As defined in 10 CFR 20.201(a), "survey" means an evaluation of the radiation hazards incident
to the production, use, release, disposal, or presence of radioactive
materials or other sources of radiation under a specific set of
conditions.
Contrary to the above, during the period of February and March 1968,
several workers entered the drywell torus room and worked in areas in
which the radiation . fields had not been properly surveyed to ensure
compliance with 10 CFR 20.201 and 10 CFR 20.202.
The dose rates in the
work area were later found to be higher than expected.
This resulted in
radiation exposures to unmonitored areas of the workers bodies that were
higher than expected.
Recponse:
GPUN concurs with the finding stated in the Notice of Violation and
wishes to again point out that this item was identified during our own
investigation.
This investigation was precipitated by the discovery,
during our daily assessment of SRD exposure readings, that worker
exposures were higher than expected.
Listed below is the reason for the
violation, the completed and planned corrective steps and the date when
full compliance will be achievea.
Reason for the Violation:
The Radiological Controls Technician on the job improperly assessed the
radiation fields as beiag fairly uniform to all major portions of the
whole body in the work area, and thereby concluded it was only necessary
to document general area radiation levels. However, radiation levels
were, in fact, nonuniform and were discovered to be significantly higher
to the head and thigh regions of individuals in some portions of the work
area than expected based on general area radiation levels.
Corrective Steps Taken and Results Achieved:
Upon oiscovering nonuniform radiation fields, work was stopped and an

exposure assessment was performed for all individuals who had worked in

the area.
Worker exposure records were adjusted accordingly.

Corrective Steps to Avoid Further Violation:
The Radiation Controls Department is performing a root cause evaluation
in. order to establish appropriate long term corrective actions.
The
evaluation and corrective action imple'aentation schedule will be
completed by June 30, 1988.
Date of Full Compliance:
Measures to achieve full compliance for the specific task, drywell torus
room cathodic. protection were completed on April 19, 1988.
Programmatic
efforts designed to prevent similar items of noncompliance in the future
will be scheduled by June 30, 1988. These efforts will be completed
prior to August 30, 1988 and will be available for NRC review.
