Justification for Operability:
No. 12 Auxiliary Feedwater Pump On October 23, 1987, the Number 12 Auxiliary Feedwater Pump (#12 AFWP) throttle valve tripped on overspeed during the conduct of Surveillance Test Procedure, STP 0-9A-1 (Auxiliary Feedwater Actuation System ( AFAS) Monthly logic Test).
In further testing over the following seven days, numerous turbine trips occurred raising concerns regarding the operability of that pump.
These trips were apparently caused, in part, by condensation in the steam supply lines and wear of the throttle control system.
On October 30,1987, you implemented a revised procedure under which the pump would be idled by steam at 1000-1200 rpm in order to keep the inlet steam piping warm and free from condensation, and you declared the pump operable.
Based on our concerns regarding your basis for this operability determination, a telephone call was conducted with Mr. J. R. Lemons of your staff to discuss this matter.
Based on that telephone call and on the commitments documented on Reference B, we understand that, until the question of reliability for the
- 12 AFWP is resolved, you will take or have taken the following actions:
1.
Operate #12 AFWP at about 1000-1200 rpm to ensure it is properly warmed and ready for startup and automatic flow initiation, if needed.
2.
Conduct a daily start of #12 AFVP, simulating demand of the pump without operator action to control speed, etc., and communicate results to NRC Region I office.

3.
Assign an additional
- operator, above minimum staffing requirements, trained and qualified to operate the AFW system in its current condition, who will monitor performance of the tests and transient conditions resulting in AFW pump demands.
4.
Implement procedures which specify how to correct an overspeed condition or governor hunting problem on the pump.
5.
Provide guidance on the response time for the dedicated operator in the event that an AFW pump demand is received.
6.
Provide dedicated communications between the control room and the dedi-cated operator via a headset in the AFW pump room.
7.
Initiate an orderly shutdown of Unit 1 in accordance with Technical Specification 3.7.1.2, Action Statement a.2.(b) if reliability has not been sufficiently improved by Friday, November 6, 1987 at 07:33 a.m.
"Sufficiently improved reliability" is understood to mean that the #12 AFWP is capable of being started and operated under simulated AFAS conditions, the pump having been idle (i.e., not rotating) for at least four hours with no operator action with respect to the pump, including its steam supply system, during this period, lhis letter does not preclude enforcement action should subsequent review of the circumstances surrounding the matter so dictate.
