Dear Ms. Peterson:
This letter is in reference to your department's letter dated August 29, 1991.
The letter requested changes to four separate registration certificates.
In response to your request, registration certificate number NR-0310-D-101-S has been transferred to inactive status.
The new registration certificate number is NR-0892-D-801-S.
The letter also referenced the following three registration certificate numbers: NR-0153-D-101-S, NR-0155-S-101-S, and NR-0155-D-120-S. We received a subsequent letter from your department that referenced these same three numbers. These certificates have already been amended as per the requests made in that letter.
Registration certificate number NR-0153-D-101-S was replaced by number NR-0155-D-124-S dated July 28, 1995. NR-0155-S-101-S and NR-G155-D-120-S were reissued with the same registration certificate numbers on July 21, 1995, and July 28, 1995, respectively.
Copies of these certificates are enclosed for your reference.
Please review these registration certificates in their entirety and notify us immediately if there are any errors or omissions.
