NRC RESIDENT INSPECTION REPORT 50-254:265/97006(DRP) AND

NOTICE OF VIOLATION

Dear Mr. Kraft:

This refers to the inspection conducted on March 18 through May 5,1997, at the

Quad Cities Nuclear Power ' Station. The purpose of the inspection was to determine

whether activities authorized by the license were conducted safely and in accordance with

.

NRC requirements. During this period routine resident inspections were performed. Based

!

on the inspectors' findings, your conduct of activities was generally characterized by

safety-conscious operations. At the conclusion of the inspection, the findings were

discussed with those members of the Quad Cities staff identified in the enclosed report.

Based on the results of this inspection, the NRC has determined that violations of NRC

requirements occurred. The violations are cited in the enclosed Notice of Violation

(Notice).

The inspectors noted good control and oversight of major plant activities. Unit 2 fuel off-

load was discontinued during the Unit 1 shutdown evolution to reduce the potential for

distraction to the operating crew. Management performed control room overviews during

startups and shutdowns.

The inspectors identified four violations pertaining to operator performance errors,

document control, records, and corrective action. The operator performance violation is a

concern because additional operator errors have been identified in recent inspection

reports. The violations in document control and records are a concern because the

potential existed for operators to utilize the wrong procedure revisions. The violation in

corrective action is a concern because your staff failed to identify and take appropriate

actions to evaluate and correct the degraded condition of a safety-related room cooler.

This corrective action violation resulted in operating with a degraded train of core spray for

an extended period of time.
