LP&L RESPONSES TO EMERGENCY PREPAREDNESS PROGRAM DEFICIENCIES

DEFICIENCY NO. 8723-01:

Development of the exercise scenario was deficient in that inconsistencies

and lack of appropriate scenario data adversely affected the demonstration

of some exercise objectives.

RESPONSE:

LP&L has implemented a program of independent reviews in the exercise

scenario development process in an attempt to identify and correct scenario

inconsistencies.

Independent reviews of future exercise scenarios will be

conducted by the Waterford 3 Quality Asaurance Department or the Waterford

3 Nuclear Operations Support and Assessments Department and comments

resolved prior to the conduct of the exercise.

LP&L's planned use of the Waterford 3 Simulator for the development of

future exercise scens-ios should also serve to assist in alleviating

scenario development problems.

DEFICIENCY NO. 8723-02:

Initial habitability determinations for the OSC were not adequate.

RESPONSE:

The exercise problem with inadequate habitability surveys in the

Operational Support Center (OSC) has been reviewed with Health Physics

Department personnel.

Proper response measures namely, completion of

habitability surveys in a timely manner, proper utilization of the

continuous air monitor, were addressed.

Emergency response organization personnel training lesson plans for the

Radiological Controls Coordinator and Health Physics technicians responding

to the OSC will be revised to reflect emphasis on the determination of

initial and continuing habitability of the 050.

This will be completed by

March 1, 1988.

DEFICIENCY No. 8723-03:

Procedures for gas-free habitability survey were not followed for simulated

entries into the reactor containment.

RESPONSE:

The problem related to the failure to follow procedures for gas-free

habitability surveys during the exercise has been reviewed with Health

Physics Department personnel. Proper response measures (including sampling

of containment atmosphere prior to entry, use of SCBAs for all entries

until habitability surveys indicate protective measures may be relaxed)

were addressed.

Emergency response organization personnel training lesson plans for the

Radiological Controls Coordinator, Health Physics Coordinator, Health

Physics Coordinator Assistant and Health Physics technicians responding to

the OSC will be revised to reflect emphasis on the need to properly assess

the environment of confined spaces during emergencies.

This will be

completed by March 1, 1988.

DEFICIENCY NO. 872?-04:

A repair team failed to comply with teentry requirements to stay with

health physics personnel.

RESPONSE:

The requirement to stay with Health Physics personnel during reentry

activities in an emergency has been reviewed with site personnel in the

December plant safety meetings. This item har also been reviewed with

Health Physics Department personnel.

Emergency response organization personnel training lesson plans for the OSC

Supervisor, Radiological Controls Coordinator, OSC Manpower personnel end

Health Physics technicians will be revised to reinforce this requirement.

This will be completed by March 1, 1988.
