Development of the exercise scenario was deficient in that inconsistencies and lack of appropriate scenario data adversely affected the demonstration

.of some exercise objectives.

RESPONSE

LP&L has implemented a program of independent reviews in the exercise scenario development process in an attempt to identify and correct scenario inconsistencies. Independent reviews of future exercise scenarios will Fe conducted by the Waterford 3 Quality Assurance Department or the Waterford 3 Nuclear Operations Support and Assessments Department and comments resolved prior to the conduct of the exercise.

.LP&L's planned use of the Waterford 3 Simulator for the development of future exercise scenarios should also serve to assist in alleviating scenario development problems.

DEFICIENCY NO. 8723-02:

Initial habitability determinations for the OSC were not adequate.

RESPONSE

The exercise problem with- inadequate habitability surveys in the Operational Support Center (OSC) has been reviewed with Health Physics Department personnel. Proper response measures namely, completion of habitability surveys in a timely manner, proper utilization of'the continuous air monitor, ware addressed.

Emergency response organization' personnel training lesson plans for the Radiological Controls Coordinator and Health Physics technicians responding to the OSC will be revised to reflect emphasis on the determination of initial and continuing habitability of the OSC. This will be completed by March 1, 1988.

DEFICIENCY NO. 8723-03:

Procedures for gas-free habitability survey were not followed for simulated entries into the reactor containment.

RESPONSE

The problem related to the failure to follow procedures for gas-free habitability surveys during the exercise has been reviewed with Health Physics Department personnel. Proper response measures (including sampling of containment atmosphere prior to entry, use of SCBAs for all entries until habitability surveys indicate protective measures may be relaxed) were addressed.

A repair team failed to comply with reentry requirements to stay with health physics personnel.

RESPONSE

The requirement to stay with Health Physics personnel during reentry activities in an emergency has been reviewed with site personnel in the December plant safety meetings. This item has also been reviewed with Health Physics Department personnel.

Emergency response organization personnel training lesson plans for the OSC Supervisor, Radiological Controls Coordinator, OSC Manpower personnel and Health Physics technicians will be revised to reinforce this requirement.

This will be completed by March 1, 1988.

Dear Mr. McGurk:

This letter is to inform you that the University of California at Berkeley has decided to 1 discontinue operation of the Berkeley Rescatch Reactor as of 1 January 1988.

We intend to decommission the reactor facility and after termination of the Nuclear Regulatory Commission (NRC) License R-101 have it released by the NRC for unrestricted use. The reactor room, after being released for unrestricted use, will be used as a Nuclear Engineering teaching and research laboratory. Operations involving radioactive matenals will be under the authorization of our State Radioactive Materials License 1333-62 or under a NRC Special Nuclear Material license.

We will keep you informed about the progress of the decommissioning operations. If you have any questions regarding this matter, please contact Mr. Andns Peterson, our Radiation Safety Officer. He may be reached at (415) 642-6166.
