Subject: Inspection No. 50-219/97-39
A reactive inspection was conducted on November 2-6, 1987. The inspection was
conducted to review the circumstances leading to the unauthorized entry into the
drywell made by four contractor workers on October 13, 1987. A review of the
sequence of events and the associated documentation identified three violations
of the station's Technical Specifications. Details are provided in the enclosed
Appendix A and inspection report. You are required to respond to the violations
in accordance with the instructions contained in the enclosed Appendix A.
The response directed by this letter and enclosed Notice is not subject to the
clearance procedures of the Office of Management and Budget, as required by the
Paperwork Reduction Act of 1980, PL 96 511.
