NOREG-0525, VOL. 2, REV. 5 JULY 1997 ABSTRACT The Safeguards Summary Event List (SSEL), Vol. 2, Rev. 5, provides brief summaries of several hundred safeguards-related events involving nuclear material or facilities regulated by the U.S. Nuclear Regulatory Commission (NRC) which occurred and were reported from January 1, 1990, through December 31, 1996. Because of public interest, the Miscellaneous category includes a few events which involve either source material, byproduct material, or natural uranium which are exempt from safeguards requirements.
Events are described under the categories of Bomb-related, Intrusion, Missing and/or Allegedly Stolen, Transportation related, Tampering / Vandalism, Arson, Firearms, Radiological Sabotage, Nonradiological Sabotage, and Miscellaneous.
The information contained in the event descriptions is derived primarily from official NRC reporting channels.

INTRODUCTION The Safeguards Stasmary Event List (SSEL) provides brief summaries of several 1
hundred safeguards-related events involving nuclear materials or facilities regulated by the U.S. Nuclear Regulatory Commission (NRC). The SSEL is comprised of two parts. Vol. 1, issued July 1992, summarizes events that occurred and were reported from pre-NRC through December 31, 1989. Vol. 2, Rev. 5, issued July 1997, summarizes events tfiat occurred January 1,1990, through December 31, 1996. Because of public interest, the Miscellaneous category includes a few events which involve either byproduct material, source material, or natural uranium which are exempt from safeguards requirements.
I As the list is intended to provide a broad perspective on the nature of j
safeguards incidents in the licensed nuclear industry, both unusual and routine events are described. Events have been placed in the several categories described below. Each listing is included in a single category unless it is transportation-related, in which case it is cross-referenced in the Transportation category.

Bomb-Related: Events concerned with explosives or incendiary devices.
Egglion A: Events in which a bomb or explosive material was located or an explosion occurred at a licensed facility.
Section B: All other bomb-related ev2nts (e.g., hoax bomb threats).

- ntrusion: Incidents of attempted or actual penetration of a dacility's barriers or safeguards systems.
i III. Missine or Allerediv Stolen: Events in which safeguarded material was stolen, alleged to be stolen, discovered missing, or found.

Each incident listing includes the appropriate date and location and a brief description of the event. Bomb threat. Hoax. At 12:38 a.m., the 911 operator received a telephonic threat, which originated from a pay telephone north of the plant, from an anonymous female who stated that six bombs would go off "any minute now."
At 12:55 a.m.,
the operator received a second call, made from a pay telephone south of the plant, from an anonymous female who stated that the devices would explode in 15 minutes.

Searches were conducted, and the sheriff's department made copies of the 911 tapes for State and Federal law enforcement agencies and allowed site security management to listen to the tapes.
Resolution: The search results were negative. The licensee determined that the threat was noncredible.

Event:
Bomb threat. Hoax. At 12:45 a.m., the Ocean County Sheriff's Department received a 911 telephene call from an individual at a pay telephone booth who said that several bombs were placed around Oyster Creek. The Ocean County Sheriff's Department analyzed the taped call, and they believe the caller is the same person who had made the threat on May 5, 1996 (See IB-96-01).
Action:
The licensee searched vital areas and tastituted their bomb threat l
procedures as a precautionary measure.
Resolution:

Event:
Bomb threat. Hoax. At 10:46 p.m., the Ocean County Sheriff's Department received a 911 telephone call from an individual who stated that the plant was going to explode. The Ocean County Sheriff's Department said that the call was by the same person who called on May 5,1996, and on July 27, 1996 (See IB-96-01 and IB-1 96-02).
Action:
The licensee searched vital areas and instituted their bomb threat procedures as a precautionary measure.
Resolution-The search results were negative.
The licensee determined that the threat was noncredible.

Event:
Bomb threat. Hoax. At 4:35 p.m.,
the Ocean Side Police Department received a telephone call from a person, identifying himself as "the Supreme Being," who stated that an " insurance policy package had been left at San Onofre."
Action:
The licensee conducted a search of the vital areas.
Resolution:
The search of the vital areas was negative. A bottle containing a note was found in front of the main gate which was closed and locked.
The note, however, did not mention anything about bombs.
The licensee determined that the incident was a noncredible bomb threat.

The NRC resident inspector was informed that the licensee had j
experienced vandalism associated with the communications system; j
headpieces and mouthpieces had been taken or damaged.
Action:
Extra observance of other plant systems was initiated, and an j
investigation was conducted.

Resolution: The results of the investigation were inconclusive. No perpetrator was identified. No subsequent damage was done.

Event:
An employee was discovered in the act of vandalizing one office,
,h three work cubicles, and a kitchen area in the administration building, located inside the protected area.
Action:
The San Luis Obispo County Sheriff's Department was notified, and a deputy responded to the site and placed the individual in protective custody.
Resolution: The individual had submitted a notice of resignation to the licensee the previous week, and the employee's last scheduled work day was June 25, 1996. The licensee determined that the employee's outburst was not job-related. There was no vandalism of any plant e All the vandalism took place inside administrative offices.quipment.

Event:
Clue was discovered in the lock core of an entry door into the Unit 2 control element drive mechanism control system room, and the padlock on a second access door to the room had also been vandalized.
Further inspection revealed that eight additional padlocks and one additional door lock had also been glued.
Action:
Plant security was notified, and an expanded search was conducted to include a check by operations of locked valves (excluding those in the containment building, annulus, or high radiation areas.)

Clue was discovered to have been placed in three hot shutdown panel key lock switches disabling their use.
Action:
All other key lock switches were checked for tampering, and an investigation was initiated.
Resolution: No additional examples of tampering were identified in either key lock switches or valve locks. The licensee took immediate steps to secure the plant and prevent similar acts and offered a $10,000 reward for information leading to the arrest and conviction of the individual responsible for the tampering. No perpetrator was identified. The investigation was subsequently closed.

Event:
At 5:05 a.m., a security guard discovered wire lodged in the key mechanism of two closed padlocks for the Units 1 and 2 reactor building.
Action:
The affected padlocks were replaced, and all other vital and protected area security key padlocks were inspected.
Resolution: No other tampering was identified. No perpetrator was identified.

Event:
Fluid was discovered in a valve operator limit switch compartment while maintenance workers attempted to reconnect the operator to a main steam line valve following operator refurbishment.
Action:
An investigation was initiated to determine the potential source and the cause of the fluid's presence.

security rechecked the same doors and discovered that wire had been inserted into the door locks causing keys to jam and making the locks inoperable.

workers were searched for traces of wire.

Upon being relieved of fire watch duty by a nonsecurity individual, a security force member, who had been issued a loaded shotgun in a came while temporarily manning the fire watch post, left the area without the weapon. Shortly afterwards, the relieving fire watch

employee noticed the unattended weapon and notified security.
l Action:
Security personnel assumed custody of the shotgun. The weapon was inspected and a determination was made that no discharge or tampering had occurred.
Resolution: The security officer responsible for the incident was disciplined.
The practice of issuing contingency response shotguns to temporary postings (such as fire watches) was reviewed, and the practice was discontinued.

While an officer was returning his weapon, it accidentally went off, sending a bullet through a wall.

The licensee conducted an investigation.
Resolution: The investigation revealed that the officer inadvertently failed to follow the proper procedure for unloading his weapon at the end of his shift. There were no injuries and no damage to any equipment caused by the bullet.

Event:
While unloading his weapon during a routine handgun inspection, a security officer found that the last round was lodged in the weapons chamber (with the primer end facing the barrel). He attempted to dislodge the round using a barrel cleaner. The barrel cleaner struck the primer and caused the weapon to discharge.
Action:
Security test fired the weapon at a firing range and determined that there were no defects or damage to the weapon.
Resolution: Refresher training was provided to all security force members on the proper procedures for identifying weapon malfunctions and on what should be done in the event of a malfunction.

A.22-caliber revolver, loaded with six bullets, was found in the tote bag of a technical employee during entry search.
Action:
An investigation was conducted.
Resolution: The licensee determined that there was no malevolent intent.

A.38-caliber revolver was accidentally discharged by a security officer while manning his post. The bullet hit the floor, and fragments chipped the lens of the officer's eyeglasses and wounded another officer's leg.

Resolution: The licensee determined that the material from which the lighter was made was very difficult to recognize on the X-ray machine. The incident did not pose a threat.

Materials licensee bomb threats. At 2:30 p.m., the materials licensee informed NRC Region I that two bomb threats had been received at the facility, both directed at buildings housing licensed nuclear material. The first occurred at 7:30 a.m., when a note was found in an elevator which indicated that a bomb was set to go off at 12:30 a.m.
The second occurred at 9:35 a.m., when a telephone call was received from an individual who indicated that a bomb was set to go off in five minutes. On the same day, the licensee also informed Region I that two prior threats involving administrative buildings had been received during the week of July 22, 1996.

After a roughly 151 percent increase in events reported between 1978 and 1979 (the result of a change in reporting requirements an increase in events l
reported following the TMI March 1979 occurrence,,and contentious licensing hearings), totals reported by year declined soucvhat and remained relatively stable until 1986 (96*) which experienced a 43 percent increase over 1985 (67).
Events reported remained stable in 1987, increased in 1988 (137) and l
1989 (148), declined by 30 percent in 1990 (103) from 1989, but increased by l
48 percent in 1991 (152) from 1990, due to an increase in bomb threats and l
misec11aneous threats reported in the first few months of 1991 during the l
Persian Gulf crises, as well as an increase in events involving safeguards j
information and contraband.
During 1992, the 94 events reported reflected a l
decline of 38 percent from 1991, primaril because of a drop in bomb-threats i
and miscellaneous events reported.
In 19 3, the 116 safeguards events reported represented an increase of 23 percent from 1992 as a result of an increase in firearms events reported.
In 1994, there were 118 events reported, reficcting an increase of 1.7 percent over 1993. The rise was attributable to the increase in the miscellaneous category; all other l
categories of reported events declined.
In 1995, there were 58 events reported, a 51 percent dro miscellaneous categories. p over 1994, largely in the firearms and In 1996, reported events declined by 45 percent from 1995; only 32 events were reported, an all-time low in the 21-year reporting period. Although reactor safeguards event 4 declined 29 percent in 1990 (100) from 1989 (142), there was an increase of 50 percent in 1991 (150) over 1990 for the reasons stated in Figure 1.
During 1992, there were 94 reactor safeguards events reported, reflecting a decline of 37 percent from 1991, primarily in bomb threats and miscellaneous events reported. Reactor safeguards events reported in 1993 rose to 116, an increase of 23 percent over 1992, attributable to the increase in firearms events reported. In 1994, there were 118 events reported.
As stated in Figure 1, the small increase of 2 events (1.7 percent) is attributable to the rise in the miscellaneous events category.
During 1995, there were 50 reactor safeguards events reported, reflecting a decline of 58 percent from 1994. The decline is due to a decrease in the firearms and miscellaneous categories of events reported.
Reactor safeguards events reported during 1996 (31) declined by 38 percent from 1995. As stated in Figure 1, the decrease in events reported in 1996 from 1995 may be attributed to the licensees' greater understanding and full implementation of the new reporting requirements.

Ilowever, 1991 (120) increased by 30 percent over 1990, for some of the reasons stated in Figure 1.

Bomb threats, which declined considerably after reaching a peak of 94 in 1979, reached a low level in 1990 (11), a 56 percent drop from 1989 (25). Of the 11 events in 1990, 4 occurred at a facility granted a full power license in March 1990. Ilowever, there was a 191 percent increase in 1991 (32) over 1990; be attributed to the crisis in the Persian Gulf.g January and February and mayDuring 1992, the 16 (50 percent) of the 32 were received durin related events versus 32 for 1991, a decrease of 63 percent. Three power reactor facilities received 2 threats each through the 3rd quarter accounting for 50 percent of the threats received, and 1 threat was directed at a nonpower During 1993, the number of bomb-related events reported reactor facility (.12), all at power reactor facilities. Bomb-related events remained stable reported reached an all-time low in 1994 (9), a 25 percent drop from 1993, and one facility accounted for 3 of the reported events. With the exception of 1980 (14), there have been relatively few intrusion events. There were no intrusion events in 1983 and none for the period 1986 through 1992. During 1993, the first reported intrusion event since 1985 occurred at a reactor site. This 1993 event led to the Nuclear Regulatory Commission's decision to modify the design basis threat for radiological sabotage, 10 CFR 73.1, to include the use of a land-based vehicle by an adversary. During 1994, there were no intrusion events reported. One inadvertent intrusion into a protected area of a reactor facility occurred during 1995.
There were no intrusion events reported during 1996.

This class of events has been relatively stable even though there has been an increase in the number of regulated facilities. There was an increase in events reported of 86 percent in 1989 (13) vs. 1988 (7), but a decline of 85 percent in 1990 (2) from 1989. There was no change in the number of tamperin5 and vandalism events reported in 1991 (2) or 1992 (2), but there was a 50 percent increase in 1993 (3) in the reported events of this type, 2 of which occurred at the same facility.
In 1994, the tampering and vandalism events reported (2) declined by 33 percent to the 1990, 1991, and 1992 level. There were no tampering and vandalism events reported during 1995.
During 1996, there were 6 tampering and vandalism events reported in this category; 2 each at 2 facilities and 1 each at 2 other facilities.

After rising steadily, increasing by 100 percent in 1988 (56) vs. 1987 (28), by l
7 percent in 1989 (60) over 1988 and 20 percent in 1990 (72) from 1989, there was a decline of 19 percent in 1991 (58) and a further decline of 10 percent in 1992 (52) in firearms reported events.
It should be noted that from 1987 to date, there has been an increase in events reported involving ammunition detection (no weapon involved). Of the 72 firearms-related events reported for 1990, 43 (60 percent) involved weapons and 29 (40 percent) ammunition only; in 1991, 31 (53 percent) involved weapons and 27 (47 percent) ammunition only; in percent) pons and 30 (58 percent) invc1ved ammunition 1992, 22 (42 percent) involved wea involved weapons and 37 events (51 percent) only; and in 1993, 35 (49 involved ammunition only.
In 1994, firearms events reported (63) declined by nearly 13 percent from the previous year (72). Thirty-five (56 percent) involved weapons and 28 (44 percent) involved ammunition only. During 1995, fireanas reported (25) declined by (60) percent from 1994, due to the reasons stated in Figure 1.
Of the firearm events reported, 16 (64) percent involved weapons and 9 (36) percent involved ammunition.
During 1996, there were 10 firearms events reported, a 60 percent decline from 1995. All of the reported events involved weapons.

This category includes not only safeguard events which do not properly fit into any of the categories defined in the document, but also covers events which involve persons or materials not subject to NRC srfeguards regulations, but which are of interest to the Commission, the industry, and the public.
There was a rise in the miscellaneous events reported of 129 percent in 1988 (32) over 1987 (14), a rise of 47 percent in 1989 (47) over 1988, a decline of 64 percent in 1990 (17) from 1989, but a marked increase of 253 percent in 1991 (60). Once again, significant numbers of events occurred in Januar February of 1991 and may be attributed to the Persian Gulf crises. y and The 1991 high of 60 events was followed by a decline of 55 percent in 1992 (27).
Miscellaneous events reported remained stable during 1993 (27).
In 1994, the miscellaneous events reported (43) increased 59 percent over 1993. Twenty events (46 percent) related to safeguards information (151993); 5 (12 percent) to contraband other than firearms (6 1993); 3 (7 percent) to general threats (4 1993); and 15 (35 percent) to other types. Note that the miscellaneous category of events reported is the only category of events which increased in 1994; all other categories declined. During 1995, the 20 miscellaneous events reported reflected a decline of 53% from the prior year, due to the reasons stated in Figure 1.
No events related to safeguards information were reported.
During 1996, there were 12 miscellaneous events reported, a 66 percent decline from the previous year. Of the 12 events, I was related to safeguards information.

The Safeguards Summary Event Ust provides brief summaries of hundreds of safeguards 4 elated events involving nuclear material or facilities regulated by the U.S. Nuclear Regulatory Commission. Events are described under the categories: Bomb-related, intrusion, Missing / Allegedly Stolen, Transportatiorwelated, TamperingNandalism, Arson, Firearms-related, Radiological Sabotage, Norwadiological Sabotage, and Miscellaneous. Because of the public interest, the Miscellaneous category also includes events reported involving source material, byproduct material, and natural uranium, which are exempt from safeguards requirements.
