Unit 1 January 1 through January 31, 1986 f

I. Narrative Summary of Operating Experiences The unit began the month at 100% power.

On January 2, 1986, while investigating a 30 MW (electrical) reduction in generator output,.a steam leak was observed on the north side of the condenser. Further investigation revealed that a crossaround steam. relief valve had prematurely lifted and steam was blowing from a breach in the relief valve tail pipe. Reducing power caused the relief valve to re-seat; however, a rapid reduction in condenser vacuum.and an increase.in off-gas flow resulted. Power was immediately restored and STP 27.4 (Turbine Trip from 100% Power) was performed satisfactorily placing the unit in the shutdown mode. While shutdown, a

relief valve in the crossaround piping and associated bellows piping to the condenser were repaired. Unit startup commenced on January 9.

On January 10, after satisfactory testing of the crossaround steam relief valves, the generator was synchronized. Startup testing continued with the l

completion of STP 25.2 (Full Closure of Fastest Main Steam Isolation Valves) and STP 23.4 (Loss of Feedwater Heating).

Performance of a control val e exercise. surveillance test revealed that the #4 main turbine control valve would not fully close. <

s On January 13, the unit was manually scrammed to investigate the Turbine control Valve #4 (TCV #4) failure.

TCV #4 failure.was the result of a bolt vibrating loose from a stop valve and wedging itself in the TCV #4 preventing it from going closed. The bolt in question has a hexagonal socket head bolt which is recessed into the stop valve cap. The bolt was replaced,. torqued and the cap was peened over in four places to hold the bolt in place.

All of the stop valves were checked to assure that the valve cap retaining bolts were properly torqued and peened into place. Startup began on January 20 and the generator was synchronized the same day.  !

On January 22, during power ascension with the "A" and "B" Reactor Feedpump Turbines (RPPTs) at rated speed, and the "C" RPPT controller in automatic, the "C" controller did not bring the "C" RPPT up to rated speed. The "C" manual speed control was lowered from its high speed stop to take control from the "C" controller and at this time, the controller for "C" failed low, resulting.in.a recirculation pump speed reduction to 75%. Troubleshooting of "C" RFPT controller resulted in recalibration of "C" RFPT controls.

Feedwater transmitters were calibrated and feedwater level control placed back into its normal three element control mode.

The unit ended ' the report period at 100% power.  ;

II. Challenges to Relief Valves During this month, the main steam system safety-relief valves were used to control reactor pressure during the reactor cooldown on January 2, 1986 due to loss of condenser vacuum.
