e Office of Nuclear Reactor Regulation Items of Interest Week Ending April 25, 1997 Maine Yankee On April 21. 1997. Maine Yankee began around the clock outage work. For the past two months, the licensee has reviewed more than 5000 work orders to identify the outage scope and to develop work plans. A] proximately 2500 work orders have been identified within the outage scope. T1e licensee continues to project a restart date in mid-July.

[ The current outage scope does not include about two dozen engineering projects that have not been fully defined. These projects include additional

! requirements for the inservice testing and motor-operated valve programs. The l licensee is near completion of a revised high energy line break analysis for the turbine building. The facility was originally licensed assuming that the largest break, for which the building walls are blown out, was limiting. The revised analysis will address smaller breaks for which the building remains l intact, possibly resulting in a harsh environment. The results of the analysis will dictate what design changes will be needed to ensure that the j equipment in the turbine building is appropriately qualified. The licensee should have the scope of the engineering outage work identified by late May.

Seabrook Station Unit 1 -- Procerty Tax Liens The towns of Seabrook and Hampton Falls, NH have served notice of intent to record tax liens against Seabrook Station because aggregate tax bills have not been paid in full. The Town of Seabrook's lien took effect April 18. 1997, and Hampton Falls' took effect on April 21, 1997. The Town of Hampton has not yet issued notice but is expected to do so. Each town issues an aggregate tax bill to North Atlantic Energy Corporation, et al. Great Bay Power Cor3 oration, one of the joint owners of Seabrook Station, is in litigation wit 1 the towns seeking seaarate tax assessment and tax abatement. Great Bay has declined to pay its saare of the latest installment of taxes pending ,

-outcome of the litigation. The remaining owners have paid their allocated share of the installment.

The liens for taxes, interest, and costs are $1.455.300 and $19.300 for Seabrook and Hampton respectively. Under NH law. the towns cannot take any action pursuant to the liens until two years after the execution of the liens.

Planned Civil Demonstration A demonstration of civil disobedience is planned at Seabrook Station on April 26,1997. Twenty years ago on April 30. 1414 persons were arrested when more than 2000 marched onto the Seabrook site to halt construction. The event l is being portrayed as a non-violent demonstration of civil disobedience. The l

event has been given free air time by the local media and flyers have been l

widely distributed at area businesses. The event is scheduled to start at 9:00 a.m. in the parking lot of the Seabrook Race Track with 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> of non-l violence training. speeches followed by a march to the south gate of the Seabrook Station. At 3:00 p.m. there will be more speeches followed by the civil disobedience.

The event is organized by "The Forces of Nature" and includes the Seacoast Antipollution League and the Clamshell Alliance. There is no information as l to how many persons are expected to attend.

Station security has been working with the local police in preparation for the event. Starting today, security patrols will be increased, and all persons entering the property will require badges. The licensee held a press briefing on April 23 and intends to invite media representatives onto the site prior to the event to observe from the inside.

Dereculation Initiative The New Hampshire Public Utilities Commission (NHPUC) has suspended most of its utility deregulation plan following legal challenges from Public Service of NH, Northeast Utilities, and several consumer and environmental groups.

The NHPUC plans to review the challenged portions of the plan and has promised rehearings starting next week. A temporary restraining order against the plan has been issued by a federal court.

Oconee Nuclear Station Units 1. 2. and 3 i l

On April 21, 1997, at approximately 10:45 p.m. , indications cf ircrrased .

leakage from the primary system into the reactor building (c,.Mc wat) on Unit 2 were observed. The leak rate was calculated to h- 2.% 4lA. Normal leak rate is only a few tenths of a gpm. In addition, reactcr ouilding I

radiation monitors showed a slight increese in gas and particulate activity.

A containment entry revealed a spraying leak in the cavity area near the 2A1 ,

reactor coolant pump and appeared to be from the high pressure injection (HPI) connection to the reactor coolant system (RCS) cold leg. It could not be i l

better defined because the water impinging on the surrounding RCS piping was l being turned to steam. Unit 2 shutdown rias started at 3:52 a.m. on April 22 l (leak rate at this time was 3.41 gpm), while the licensee continued to closely monitor the leak indications. When the leak rate increased to 10 gpm, an Unusual Event was declared in accordance with the Emergency Plan at 4:00 p.m.

and exited at 8:32 p.m., when indications showed that the leakage was sufficiently below 10 g]m. With reactor pressure approximately 250 psig, an '

entry into the reactor Juilding revealed that the leak was from the weld in the 2.5-inch HPI line connection to the RCS and extended ap3roximately half-way around the pipe. The reactor is in cold shutdown and tie licensee is analyzing the failure. NRR and Region II staffs are following the licensee's activities.

LaSalle Units 1 and 2 -- Pro.iected Restart Extended LaSalle has moved its projected restart date from September 1997 to May 1998 (both units have been shutdown since September 1996). This is an economic i decision based on a substantial increase in the operating budget that would have to be required to return the lead unit to operation by the projected date l of September 1997. The licensee has determined that because they will not be APRIL 25, 1997 ENCLOSURE A l

operating for this summer's peak, they will focus on returning to operation for next year's peak season. The cutbacks will involve the elimination of 450 j of the 700 contractors at LaSalle. No Comed employees will be affected. The i licensee will continue to implement its restart plan and there has been no  ;

change to the scope of activities to be completed prior to restart.

I Ouad Cities Units 1 and 2 and Dresden Units 2 and 3 On April 1,1997. Quad Cities identified cracks on an auxiliary switch mounted  :'

in a 4 Kv Merlin-Gerin breaker which resulted in the switch physically separating from its mounting. The inspection of 14 other breakers resulted in finding 12 breakers with cracked auxiliary switches. On April 9,1997, the licensee declared all Merlin-Gerin breakers inoperable and commenced shutdown of Unit 1. Unit 2 was already down for a refueling outage. On April 10.

i 1997, Comed shutdown Dresden Unit 2 after discovery of similar failures in i their Merlin-Gerin breakers. Unit 3 is also shutdown for a refueling outage.

These breakers were supplied to Comed for Quad Cities and Dresden originally  :

through Golden Gate Switchboard and then through Pacific Breaker Systems over i the past three years. Most of these breakers are installed in locations to feed or receive power to or from the station main buses: (bus 13, 13-1, 23, -

23-1, 14, 14-1, 24 and 24-1). A Comed investigation team has provided a preliminary finding that concluded that the cracking of the phenolic switch contact block is due to unevenness of the switch mounting plate and flexing of  ;

the plate during operation of the breaker.

After testing some of the suspect breakers at Comed's System Material Analysis Department (SMAD) in Bollingbrook, Illinois, the licensee in consultation with ,

the breaker manufacturer proposed an interim fix by placing a nylon strap -

inside tygon tubing around each switch block and mounting plate after assuring the switch block is currently intact with no missing pieces. If the switch -

block does have missing )ieces the breaker would be replaced. Seismic testing T has been completed on a areaker with a cracked switch and a nylon strap in >

place and the licensee reports acceptable results. The root cause of the cracking; however, has not been finalized. As of April 23. 1997, all breakers j

required to be repaired on Quad Cities, Unit 1, and Dresden, Unit 2, have been completed. Plant startup for Quad Cities, Unit 1 is scheduled for April 23, 4

1997, and Dresden, Unit 2 on April 26, 1997. R-III and NRR are following this issue.

Zion Units 1 and 2 -- Decision not to Reolace Steam Generators On April 17, 1997, the licensee issued a press release stating that it will  ;

not' invest the $400 million necessary to replace Zion's steam generators. In i an informrtion bulletin to the station, the licensee stated that since the steam gene;ators have a useful life of less than 8 years and since the decision has been made not to replace them, Zion will probably be retired  !

prior to its 2013 license expiration date, and probably before 2005. Comed's  :

Chairman, James J. O'Connor, said that the decision is a result of Comed's assessment of the economic viability of its generating assets, which addresses all options, including sale, retirement or continued operation of the plants.

APRIL 25, 1997 ENCLOSURE A

Chance in Unit 2 Startuo Schedule In late March 1997 Comed announced its intention to startup Unit 2 in 100 days, which would have been in early July. On April 17, 1997, in a ,

memorandum to the Zion employees, Comed announced that there is too much  :

training and performance demonstration still remaining to be accomplished to ,

meet the 100 day goal. Dn April 18, 1997, the Chief Nuclear Officer, Tom Maiman, and the Site Vice President, John Mueller, held all hands meetings at which they announced the Unit 2 schedule change and that Comed was dropping the goal of start-up in 100-days. The licensee's current intent is to restart Unit 2 in the fall of 1997. Sustained operation of both units by summer 1998 is its long range plan.  ;

Assessment of Ooerations Personnel Comolete On March 24, 1997 Comed issued an announcement describing its program to ,

improve operations and overall site performance. The first step in this )

program was an assessment of the operations department personnel. On April 17, 1997. Comed announced the results of the assessment. A total of 187  :

employees, both management and bargaining, licensed and unlicensed, were i assessed. Some of the assessed em)loyees were not assigned to operations at  !

the time, but held licenses, and lence were evaluated, Of the assessed personnel,141 were selected 'for the operations department. The personnel not  :

selected for the operations department were either placed in a remediation l 3rogram, assigned other positions at the site outside of operations, or will 3e employed elsewhere at Comed or will be terminated. l APRIL 25. 1997 ENCLOSURE A

Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending April 25, 1997 Meetina with Canadian Atomic Enerav Cortrol Board Manaaers On April 22. 1997. managers of the Directorate of Fuel Cycle and Materials Regulation. Atomic Energy Control Board (AECB) of Canada met with counterpart managers in the Nuclear Regulatory Commission's Office of Nuclear Material Safety and Safeguards (NMSS) to discuss regulatory issues of mutual interest. i NMSS managers made presentations on current regulatory issues and plans for their resolution. AECB managers presented similar topics, and included discussion of recently-approved legislation. The Nuclear Safety and Control Act, which provides comprehensive regulatory control of nuclear materials. 1 This Act and the implementing regulations are expected to become effective in  !

1998. Following the meeting, the AECB managers met with NMSS staff members for more detailed discussions.

Trilateral Coooeration on Excess Fissile Material ,

I On April 18, 1997, a member of the Regulatory and International Safeguards Branch participated in a U.S. Trilateral Initiative working session at Sandia National Laboratories in Albuquerque. New Mexico. The Trilateral Initiative is a joint effort between the U.S., Russia, and the International Atomic j Energy Agency (IAEA) to address verification of excess weapons-origin fissile '

mdterials. During the working session, which included participants from U.S. Government agencies and national labdratories, a number of technical issues were discussed, including: attributes of the fissile materials covered by the Initiative, material container designs, aad possible verification and monitoring techniques. Issues addressed during the April 18 meeting will be discussed during the upcoming May meetings between Russian and IAEA  :

counterparts in Vienna, Austria, during which the Progress Report on the Trilateral Initiative will be finalized. '

Insoection at Precision Comoonents Corocration On April 15-17. 1997 Spent Fuel Project Office staff conducted a safety team inspection of dry storage system fabrication activities at Precision Components Corporation (PCC) in York. Pennsylvania. PCC is the fabrication vendor to Transnuclear. Inc. . for the TN-32 dry spent fuel storage system.

PCC is fabricating 14 TN-32 units for Virginia Power Company (VPC0), Surry and North Anna Plants. VPC0 staff identified a pinhole leak in the loaded TN-32.

Unit No. 1. at the Surry Plant in December 1996. While grinding out the non-safety significant weld to repair the leak. VPC0 staff found intermittent lack of fusion along 36 inches of the weld.. The empty Unit No. 2 was inspected at Surry and found to be acceptable. However. PCC found and repaired leaks in the partially complete Units No. 3 and 4. PCC performed a root cause analysis '

to establish the factors leading to the lack of fusion in the welds. PCC has instituted several weld process changes to improve weld quality, however at j the time of the NRC inspection the lack of fusion problem was not yet solved.  ;

PCC is continuing to work on the solution. The inspection team had other findings of minor safety significance. An exit meeting was held on April 17, 1997, to present the preliminary inspection findings.

Meetina with Palo Verde Recardino Dry Cask Storaae On April 23, 1997, members of the Spent Fuel Project Office staff met with Palo Verde Nuclear Generating Station (PVNGS) staff regarding dry cask storage l 1 of spent fuel under the general license provisions of 10 CFR Part 72. PVNGS l

! alans to utilize the Nuclear Assurance Corporation International (NAC) l Jniversal Multi-Purpose Canister System (UMS), a dual-purpose storage and .

transport design. The NAC-VMS design is similar to currently certified NAC-storage / transportation cask. technology. The NAC-VMS transport' application is ,

expected on April 30. 1997, to be followed by the storage application on '

June 30, 1997. '

The PVNGS presentation included licensing strategy. quality assurance ,

oversight, Jublic outreach, and the spent fuel storage implementation l schedule. 3ased on preserving full-core off-load capability. PVNGS plans to i commence loading spent fuel into the Independent Spent Fuel Storage Installation in the Year 2000. PVNGS also plans to submit an application to j i the Office of Nuclear Reactor Regulation for boron credit in spent fuel pools.

core off-load capability until the year 2002. i Deoartment of Enerav Exoloratory Studies Facility l

The Department of Energy (DOE) announced that the Tunnel Boring Machine (TBM) is 3.9 meters (12.8 feet) from completing its approximately 7800 meter (5 >

mile) excavation of the Exploratory Study Facility (ESF) and is ready to  !

! " daylight". DOE indicates that the TBM will officially daylight between 10:00 and 11:00 am Friday. April 25, in a special ceremony at the South Portal.

This activity began in September 1994 and represents a significant milestone in completion of the main loop of the ESF. Additional work will continue in  :

the ESF. including on-going scientific testing and excavating test alcoves.  ;

Videoconference on Atlas Final Environmental Imoact Statement l i

On April 18, 1997, staff from the Division of Waste Management and its  ;

contractor. Oak Ridge National Laboratory, met via videoconference with '

representatives of the National Park Service (NPS) and other Department of the  :

~ Interior staff to discuss a preliminary version of the Final Environmental .

Impact Statement (FEIS) for the proposed reclamation of the Atlas mill tailings site in Moab. Utah. NPS is a cooperating agency in the preparation of the Atlas FEIS. The preliminary version of the Atlas FEIS had been sent to I NPS for comment in late March. The NPS has expressed concern over Atlas' 3roposal to stabilize the tailings in place, preferring instead that the tails

)e moved to a location away from the Colorado River floodplain.

Prior to and during the videoconference. NPS identified general areas of

. concern. These general concerns, which include the characterization of the l tailings pile and the data on current conditions in the Colorado River, have

l been raised by NPS in the past. NPS expressed its view that the Summary and l

Conclusions section should contain a discussion of the current conditions in the Colorado River and that the section did not accurately portray the l uncertainties in some of the conclusions identified in the text of the FEIS.

NPS agreed to provide specific examples to the Nuclear Regulatory Commission and NRC agreed to consider revising the Summary and Conclusions section.

events. NUREG/CR-4674. Volume 23 was approved for publication on A)ril 20. A l total of ten 1995 events were identified as precursors. Eight of t1e ten ,

involved the unavailability or degradation of equipment or systems: the other i two involved event initiators. Events and conditions related to electric power continued to be a significant contributor to the fraction of precursor events (six precursors involved problems with electrical equipment, although none involved a total loss of offsite power). The 1995 3recursor with the highest conditional core damage probability (CCDP) was tie August 2. 1995 complex event at St. Lucie 1. which had a CCDP of 1.1x10" involving failed pressurizer power-operated relief valves, multiple failures of the stages of a reactor coolant pump seal, and other equipment problems. Beginning with the 1995 precursors. the final analysis of each licensee and made publicly available as soon?recursor was transmitted to the as it was completed.

Consequently, the 1995 Precursor Report was restructured to present a compilation of the precursor analyses which had been issued during the past year.

ANS Meetina An IRD staff member attended the ANS meeting in San Francisco on April 24, 1997 to give a presentation on " Protective Action Decision-Making."

National Radioloaical Emeroency Preoaredness (REP) Conference IRD staff attended this conference to discuss current issues in emergency response with representatives of Federal and State agencies and licensees.

The IRD Director presented an overview of NRC's response to radiological emergencies including the use of DOE facilities and response to events at gas diffusion plants.

Earth Day Observance Earth Day 1997 was observed in Headquarters on April 22, 1997. - Over 125 emaloyees visited the Earth Day Information Center set-up in the TWFN Building lo)by. Representatives from the NRC Administrative Services Center (ASC).

Montgomery County White Flint Commuter Shuttle. North Bethesda Transportation  ;

Management District and MARC Rail provided information and answered questions ,

about transportation, recycling and conservation. Interested employees were  !

also introduced to the new Council of Governments' Commuter Connections Ridematching System which can now be accessed by contacting the ASC.

Information Security Oversicht Office (1500)

I During the period April 23-24, 1997 NRC was inspected by Information Security Oversight Office (IS00) to determine NRC's compliance with Executive Order 12958 " Classified National Security Information." This was the first NRC inspection since the E0 became effective in October 1995. The IS00 inspector conducted interviews with classification officials in NRR OIP and F0IA:

reviewed approximately 40 classified documents for proper classification and markings: reviewed NRC's security awareness program required by the EO: and was briefed on NRC's progress under its declassification management program.

During the exit interview with the Deputy Director of Facilities and Security.

the inspector commented very favorably on all aspects of NRC's program and discussed the challenges NRC faces in monitoring the additional classification actions and activities from the gaseous diffusion plants. A fir.al report

Restack Proiect Phase 2 of the Restack Project (0WFN 3rd floor) is 95% complete. The floor will be occupied beginning May 7. 1997. The occupants of the third floor are the Office of State Programs, the Office of Investigations and the Personnel Satellite Office. Phase 3 of the Restack Project will begin the week of May 1 12. 1997. Phase 3 consists of the 17th floor, excluding the Chairman's suite.

The 17th floor Restack work will occur after-hours, between 7:00 p.m. and 5:00 a.m.

o Office of Enforcement Items of Interest Week Ending April 25. 1997 TVA's Emoloyees Concerns Proarams On April 29, 1997, the Director of OE and other senior NRC managers met with the Tennessee Valley Authority's Inspector General, Assistant Inspector General for Investigations and a TVA IG Auditor to discuss TVA's Employees Concerns Programs. The discussion involved the TVA's Employee Nuclear Safety and Quality Concerns Questionnaire developed by the TVA's Office of the Inspector General and the results of the TVA IG audit for both the Concerns Resolution Programs for TVA Nuclear and for the Sequoyah Nuclear Plant.

tour of the Region I radiological laboratories and equipment for a Boy Scout troop from Audubon, PA. The Scouts received explanations of the various ,

equipment and instrumentation in the laboratory, and demonstrations of a l variety of field survey instruments. The tour satisfied part of the Boy Scout  !

requirement for a Citizenship merit badge.

-Rights visited Region III on Monday April 21, 1997 to discuss management issues. This was the first of several planned visits to the regional offices.

l The team met initially with Regional Administrator. Bill Beach and his principal staff. Later a meeting was held with all regional managers and l i

supervisors. A variety of managerial and administrative issues were discussed in these sessions. In the afternoon, Ms. Norry led an "all staff" meeting discussing the new NRC reorganization. The office directors briefly discussed their organizations and areas of responsibility. This was followed by a question and answer session. l Similar visits are planned for the other regions in the coming months.
