ST. LUCIE UNIT 1 - LOSS OF 1 OF 2 OPERATING SHUTDOWN COOLING PUMPS WHILE IN MID-LOOP OPERATION (MISC ITEM #26) 4 On May 4, 1993, one of two running St.-Lucie Unit 1 LPSI pumps 1
was shutdown due to signs of pump distress.
LPCI pump bearing housing was damaged by heat.
The RCS level was at midloop level i
at the time and TS 3.9.8.2 and TS 3.1.1.3 action statements applied.
TS 3.9.8.2 requires to initiate pump repair or flood up to a greater than 23 feet above the reactor fuel.
The reactor had been refueled and operations had lowered RCS level to mid loop for steam generator nozzle dam removal and RCP seal replacement.
The unbolted recently seated reactor vessel head was removed for the floodup the reactor.
The refueling pool was flooded to 23 feet above the fuel on May 5, 1993.
During the floodup period, no RCS instabilities were noted.
Licensee Corrective Actions:
1.
Replaced the bearing housing inclusive of new bearings and pump shaft.
2.
Revised the applicable pump overhaul procedure.

Returned the pump to service prior to reducing 3.
y inventory for RCP seal replacement.
