SUBJECT: NRC INSPECTION REPORT 50-361/97-05; 50-362/97-05 AND NOTICE OF

VIOLATION

Dear Mr. Ray:

An NRC inspection was conducted February 23 through April 5,1997, ;t your San Onofre .

Nuclear Generating Station, Units 2 and 3 reactor facilities. The e Joseo report presents

the scope and results of that inspection.

During the 6-week period covered by this inspection, your conduct of activities at the San

Onofre facilities was generally characterized by safety conscious operation, sound

engineering and maintenance practices, and careful radiological work control.

One violation was identified during this inspection regarding cooling down the Unit 2

pressurizer at a rate greater than allowed by Technical Specifications, in particular, an

operator was not attentive in the performance of a surveillance requirement which led to

exceeding the maximum pressurizer cooldown rate allowed in a 1-hour peri od. In addition l

to the violation, there were four other examples of operators not being fully aware or

knowledgeable of important plant conditions or limitations. These additional examples

involved reactivity control (two examples) and shutdown cooling flow and configuration

(two examples). While the safety consequence of these individual issues was small,

together they demonstrated a weakness'in operator performance.

The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding this violation is described in detailin the enclosed report. Because we have i

completed our review of your corrective actions for this violation, you are not required to

respond to the Notico.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,

its enclosure (s), and your response will be placed in the NRC Public Document Room  ;

(PDR). To the extent possible, your response should not include any pe'rsonal privacy,

proprietary, or safeguards information so that it can be placed in the PDR without

redaction,  !
