Gentlemen:

Subject: Inspection 50-289/85-24

During the period from October 11-18, 1985, members of the NRC TMI-1 Restart

Staff conducted routine and special safety inspections of operational activ-

ities at your facility. The results of these inspections are documented in the

enclosed inspection report. At the conclusion of the inspection Mr. R. Conte,

of my staff, summarized the inspection findings for you and other members of

your staff. The findings were generally favorable; however, one area requires

your prompt attention.

Your attention is specifically directed to paragraph 3.2.6 of the enclosed

report in which we describe our observation of a worker standing on the valve

operator for an open feedwater isolation valve. Subsequent to this inspection

period, on October 19, 1985, we had a similar observation in the reactor

building on a decay heat removal system isolation valve. Such actions could

cause a plant transient and result in an unnecessary challenge to safety

systems and the operators. Based on our recurring observations, we believe

that .this is a potential attitude problem indicative of inadequate supervisory

oversight. This is similar to our finding in Inspection Report 50-289/85-22 on

the apparent violation of regulatory req'uirements for the storage of scaffolding

in safety-related areas. Collectively, we consider these observations to be

reflective of an adverse trend in your control of personnel working in safety-

related areas. Although no transient or adverse effect upon a safety-related

system resulted, we believe your immediate attention is required to reverse the

adverse trend in this area.
