The following action has been taken concerning th'e' use of a 5 millicurie cobalt 60 sealed source, Harshav No. HC-200v, a.t the Cleveland Clinic, 93rd St. and Euclid Avenue, Cleveland, Ohio hh106:

a)

On September 11, 1969, a request was sent to Mr.

R.

Brinkman, Division of Materials Licensing, Washington D.C.

20545, that our license No. 3k-6558-5 be amended to include the Cleveland Clinic as a. temporary user $ area.

The Cleveland Clinic is presently operating under General License No. 34-00466-01.

b)

The source is in our possession and will not be used at the Clinic until authorization to do so is received from the AEC.
