HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING AUGUST 30, 1985 Shoreham On August 26, 1985, the ASLB issued its Concluding Partial Initial Decision on Emergency Planning for Shoreham. The Board ruled that there is no reasonable assurance that adequate protective measures c,an and will be taken in the event of a radiological emergency at Shoreham and therefore ordered that no operating license be issued. The Board based its ruling on LILCo's lack of legal authority to implement the plan it submitted and the fact that State and County opposition to the plan has created a situation where it is not known whether the plan would be workable. The legal authority issues have been appealed to the Appeal Board, which has not yet ruled.

Sequoyah Units 1 and 2 On August 21, 1985, TVA initiated a shutdown of both units due to reports from their independent auditor (Westec) that TVA did not have sufficient documentation to prove, conclusively, that the plant's equipment is environmentally qualified. Today Region II informed NRR that TVA plans to assign 40 TVA personnel and about 30 contractor personnel to review equipment qualifications packages. TVA does to anticipate being ready for S quoyah restart prior to December 1, 1985.

NFS-Erwin Strike by OCAW Union members continues. On August 14, 1985, NFS was informed that NRC has no objections to NFS conducting limited operations of the high enriched uranium production and scrap recovery facilities. NFS introduced material into the HEU production area on August 17, 1985.

Part 21 Report NRC was notified by letter from Limitorque Corporation to Headquarters dated August 13, 1985, of potential failure of the worm shaft gear in Limitorque Actuator Size 2 Type SMB, SB, and SBD. The cause is impact loads associated with the clutch mechanism returning from manual operation to motor operation in high speed situations. This failure would prevent proper positioning of the valve in the motor drive mode. However, auxiliary handwheel operation would still be available. The problem only exists when the actuator is used with a two-pole AC motor (3600 rpm, 60 hertz or 3000 rpm, 50 hertz) or a DC motor using an actuator ratio less than 55.48 to 1. The notification results from investigation of apparently isolated failures at Calloway and Wolf Creek. These failures could only be duplicated when unusually.

Plants mentioned in the letter were: Duane Arnold Nine Mile 1, Pilgrim, Yankee Atomic, Sequoyah, Millstone, Monticello, Oconee, Limerick, Fenni 2 Brunswick, Hatch, Crystal River 3. Indian Point 3 River Bend 1, Oyster Creek, and TMI. Additionally, Atwood & Morrill Company, and Westinghouse Corporation obtained the actuators. The vendor has notified the users.

St. Lucie Plant, Unit No. 1 Earlier, on August 22, 1985, a LOCA analysis error was discovered that would have allowed St. Lucie 1 to exceed the 2200*F peak clad temperature limit during a certain LOCA scenario (a highlight was sent on August 22

, regarding this issue).

EXXON has provided results to the staff of additional analyses that

! indicate the linear heat generation rate setsoint can be raised to more than 14.0 kw/ft. As a result, the licensee las raised the setpoint to 14.0 kw/ft and has returned to 100% power.

Florida Power and Light Company is preparing a letter to provide greater t

detail with regard to this problem and its actions to fully resolve this issue.

i j Sequoyah Units 1 and 2 i '

Three Mile Island On Monday August 19, 1985 the ASLB issued a partial initial decision on j the Dieckamp mailgram issue in the Three Mile Island hearings. The 125 j page decision states that there is no evidence which imputes the integrity

of Mr. Dieckamp as a result of the mat 1 gram he sent during the accident, j j The intervenors may appeal this ASLB decision.

, Watts Bar Nuclear Plant, Units 1 and 2 Due to problems regarding welder certification uncovered by Region II

during their investigations of allegations TVA has issued a stop work i

order on welding acttvities at the Watts Bar Nuclear Plant. Problems include the back dating of welder certification cards and the updating of cards belonging to welder foremen without the individual fulfilling recertification requirements.

, Region II has concluded that certification activities at other TVA i facilities are better controlled by the applicant.

! Diablo Canyon Unit 2

, The Diablo Canyon Unit 2 full-power license was issued on August 26, t

1985. Unit 2 went critical on August 19, 1985, low-power testing is '

{ expected to be completed during the first week of September, and '

, commerical operation is scheduled to consnence in December. There are now 93 licensed commerical reactors excluding those that are in a long l term shutdown or inactive state.

Earlier studies had shown problems in the areas of aging, operability times and operator actions required to limit the severity of the design i basis accident environment, which are somewhat unique because this is  ;

t l Wolf Creek ,

At 5:00 a.m. on August 27, 1985 KG&E completed the 250 hour0.00289 days <br />0.0694 hours <br />4.133598e-4 weeks <br />9.5125e-5 months <br /> 100% power warranty run and carried out a Trip from 100% power. These tests complete the requirements established by the Kansas Corporation Commission for comercial operation. KG&E expects to declare Wolf Creek conenercially operable upon return to service on August 30 or 31.

The unit is currently shutdown to permit replacement of all eight unqualified nylon terminal blocks that were inadvertently installed as replacements for polysulfone qualified terminal blocks in the main ,

steam line isolation valves and the main feed water isolation valves.

Wolf Creek received its construction permit on May 17, 1977, its low power operating license on March 11, 1985 and its full power operating license on June 4, 1985.

Rancho Seco Nuclear Generating Station As a result of a recent INPO appraisal on the performance of some of the licensee's employees in the areas of adherence to procedures, supervisory involvement, control of maintenance activities and watchstanding practices of non-licensed operator the licensee concluded that the implementation of programs in these areas are not as effective as they should be. Therefore the licensee has committed to implement the following actions prior to restart of Rancho Seco:

1. Establish and put into effect a program to ensure a more active involvement by supervisory personne within the plant to assure that L policies and procedures are properly implemented. '

2. Develop and implement a program to more closely control maintenance activities.
3. Implement a program which upgrades the watchstanding perfor. nance of non-licensed operators within the plant.

Oconee Nuclear Station, Units 1, 2 and 3 As part of the current Oconee Unit 3 refueling outage, Duke Power '

Company (Duke) is performing an overhaul on one of the four reactor coolant pumps (RCP). Duke noted that 21 of the 32 bolts on the pump journal bearing housing were broken and believes the cause to be '

fatigue (transgranular cracking and intergranular stress corrosion ,

cracking). Duke also noted excessive wear on the stuffing box. '

Although not part of the pressure boundary, the bearing housing is in '

contact with reactor coolant water. The stuffing box
