Please find the enclosed Licensee Event Report (LER) 94-009-02 regarding unauthorized tests of Makeup Tank level / hydrogen pressure.

The original LER described the unauthorized test which occurred on September 5, 1994 while i

Supplement 1 to the LER described other concerns related to operating curve changes and reviews of calculations that occurred or were discovered subsequent to the original event. This supplement identifies a similar unauthorized test discovered to have been performed on September 4, 1994, incorporates the root causes of the related concerns along with associated corrective actions, and provides an update to previously described actions.

These corrective actions have been previously communicated to NRC via FPC Letter 3F0996-01 dated September 9, 1996 in reference to NRC Report No. 50-302/EA 95-126.
