This moming the independent Self Assessment of Zion concluded with a presentation by team members to about 180 of us. Because all of you could not be present to hear the insights from the team of industry peers, I want to provide l
my perspective to you on the message I heard.
l Although the assessrnent results were divided into such traditional areas as i
operations, maintenance and engineering, I was struck by common themes j
throughout the team's presentation. These themes should give us serious insight i
to the consequences of our oorformance. I was impressed by the commitment of the people who delivered tr.e message today - they truly delivered a no-holds-barred message. That message is:

We are not helping each other succeed. We are not learning from each 4
other or the industry - we are more interssted in blaming each other -

management, union, other work groups and also people outside of Zion than we are in operating Zion successfully. The way we treet each other is l
unprofessional and does not promote respect for each other or our jobs.

We do not approach our jobs as though we are accountable for the suocess j
of what we are doing - we have no performance measures to determine if j
we can even succeed. We are not wilung to define high standards for j
performance and then hold ourselves socountable for meethg those l
standards, instead we focus on our own problems, stay in our individual silos and do not contribute to our overall success.

Our work control processes inhibit performance and our engineering programs are ineffective. Our operating and meintenance standards for equipment reliability, cleanliness and redistion protection are far below what other plants view as acceptable. Plant equipment is suffering from our inability to operate and maintain N to meet minimal industry standards.

l Although some of us may believe that the message was directed tn managers, and much of it was, it was also strongly delivered to senior and mid-levet managers, '
l supervisors, workers - the entire workforce. The team was clear that all levels at l
Zion exhibit a lack of leadership and' accountability. We must get serious about ownership, accountability and performance improvement.

Concluding two weeks on site, the Independet Seif Am team presented its i
detaued Sadings to 200 LaSaBe employees this morning. The RM., b a puna=ry of what they, as peers, had to say about 14SaBe station.
There were several common themes in the areas ofep.Ger.s, maintenance and l
engineering. Giving immediate attention to these items is essential to the success of laSaDe. The main themes are:

generally healthy working relationship.
i We are not performing as a team. We are not loaming either from each other or l

We art not accepting accountability for the results of our work, and we are not even measuring whether or not we are successful. We tend to work within silos i

This letter is to inform you of the actions that will be taken to address key problem areas and accelerate the rate of performance improvement at LaSalle County Station.
During the period December 3 through December 13,1996, an independent Industry Safety Assessment (ISA) team conducted a comprehensive review of the performance at the LaSaile County Nuclear Station. This assessment, done at the request of Comed, was conducted by a team of industry peers, and included representatives of the institute of Nuclear Power Operations. The team noted a number of deficiencies in the areas of Operations, Engineering, and overall Management. This assessment confirmed the deficiencies identified earlier in the Service Water Sealant injection Event in June,1996, the Service Water System Inspection conducted by the NRC on September 3-24,1996, and the SALP 13 report of October 17,1996. The ISA team provided valuable insight into these issues, along with a number of suggestions for addressing them. We ar; planning to brief you and your staff on the resu!ts of the ISA in a public meeting on January 9,1997.
As you may recall, I made a strong statement at the SALP meeting on October 29,1996, that the LaSalle nuclear units would not be started up before we were ready to conduct the start up and subsequent operations in a safe and reliable manner. In keeping with that philosophy, and in consideration of the additional insight provided by the ISA team, I have canceled the start up dates of the units, and directed developraent of a restart plan that will address the following four major areas: safe plant operation, effective work management, engineering support of the plant and configuration control, and effective human interaction and performance.
