A minor change to UNC's IIerratite license is requested to more realistically describe the controls exercised over product samples sent to Mallinckrodt Che:aical Warks Analytical Laboratory.
Since MCW has an SNM licence, UNC proposes to re.nove the requirement that we will have a person assigned to the Lnboratory.

Product sampics for chemical analysis are sont to the Analytical Laboratory of Mallinchrodt Chemical Works. To assure that no moro material is sent to their facility than their licensed.
.' amount, a matorini balance log is maintained. The laboratory supervisor at liematito will keep a current log of sampics submitted including a running balance.. This control is also exercised by the responsible individual (s) at the l'allinckrodt Labs.
Title to the material submitted to LDV for analysis shall remain with United Nuclear Corporation. Health and safety responsibility for the analyses performed thoro will bo assumed by Mallinckrodt under the provisions.of their SNM license.
Unused sampic portions and analytical residuos will be returned to llomatito after completion of testing by Mallipckrodt.
