Inspection 50-289/85-19
By Memorandum and Order CLI-85-09, dated May 29, 1985, the Commission issued a
decision that lifted the effectiveness of previous orders issued in 1979 that
directed TMI-1 to remain shut down. To manage and coordinate the Region I
inspection ef forts for the restart of TMI-1, a separate organization was
established. This organization, the NRC THI-1 Restart Staff, was established
to provide increased direct inspection and monitoring and increased senior
management involvement during this period.
Between May 31 and June 28, 1985, members of the NRC TMI-1 Restart Staff
conducted a special safety inspection of activities at your facility.
The
results of that initial inspection are documented in the attached Inspection
Report. At the conclusion of the inspection, Mr. R. Conte of my staff sum-
marized the inspection findings for you and members of your staff. These
findings were generally favorable.

As a result of the inspection, however, one of our inspectors identified a
plant condition in which you were not in full compliance with your license.
The attached Appendix A, Notice of Violation, describes operator actions which,
in part, were not in accordance with your established procedure for maintaining
the correct valve lineup in a reactor coolant pump seal leakoff line.
In
addition, control room operators failed to recognize the recorder indication
of abnormally low seal leakoff flow for more than eight hours.
Failure to
identify and correct the condition prior to restart could have led to failure
of the seal with consequent challenges to both the operators and safety systems.
No response is required to the Notice of Violation because you acknowledged
this noncompliance, took immediate corrective action to be in compliance with
your procedure, and completed adequate measures to prevent recurrence as
verified by our inspector. We do not view this incident as indicative of a
programmatic problem.

This letter also responds to your letter dated June 8,1985, which described
your plans to implement License Condition (LC) 2.c.8(2) regarding shutdown of
the facility when primary to secondary OTSG 1eakage exceeds baseline by 0.1 gpm
(6 gph). Your plans were reviewed in detail as described in paragraph 5.2 of
the attached inspection report and, as noted in the report, the license condi-
tion as it stands is enforceable. We do concur, however, with your plan which
calls for further investigation and confirmation of primary to secondary leak
rate indication in excess of the LC limit to avoid undesirable thermal cycles
that may result from unnecessary shutdown of the facility. During our review
of your implementing procedure for primary to secondary leak rate monitoring,
we noted that your planned actions for the determination of actual leakage
lacked specificity.
However, we understand that your planned actions in the
implementing procedure will be re-reviewed as a result of knowledge and
experience gained through the power escalation test program, and we will
review this area again at that time.
