Enclosed is a final. report, in accordance with 10CFR50.55(e), for the problem concerning nonconformances in tube fittings which were installed by Johnson Controls, Incorporated. This problem was reported via tel-con to T. Silko of your staff on November 9, 1984. An interim report was submitted via our letter dated December 10, 1984.

Description of the Problem Unacceptable linear -indications on fittings supplied by Parker-Hannifin may have - been corrected by grinding, however, Johnson Controls Incorporated ~.did

Systems using these fittings may include the residual-

Analysis of Safety Implications
! A : detailed analysis to determine the safety impact of grinding below' the
. minimum wall thickness requirement:was not performed.
However, if the wall thickness.was reduced -beyond acceptable limits, -it.could cause the loss of -
redundant safety-related -instrumentation - thus adversely affecting the safety

of _ operations of the plant.
. Corrective Actions-1.
' Grinding in excess of 30 percent of the nominal section thickness will not be allowed; unless. specifically. directed by a Nonconfomance and Disposition Report.
-Where minimum wall violations are suspect, a
thickness verification inspection will-be performed per-procedure JC-QAS-1101.
2.
An attribute has been _added to the JCI Tubing Walkdown Checklist' requiring identification and corrective action.for fittings exhibiting potential minimum wall violations. -The acceptance criteria is stated in the Engineering and Design Coordination Report No. C03343.
