Description of the Problem Quality Assurance Category I
service water pump strainer motor. lead connections were sealed with insulating material not conforming to Electrical Installation Specification No. MP2-E061A.
This nonconformance was documented on Nonconformance and Disposition Report No. 9727.
To ensure that the approved insulating material was used to seal lead connections' of other Category 'I motors (excluding motors with terminal blocks), Field Quality Control reinspected the motors for which work had been completed and accepted. This inspection identified a nonconforming connection to the low pressure core spray pump, 2CS L*P2, which was documented on Nonconformance and Disposition Report No.10127.
Analysis of Safety Implications Under certain condi tions, the use of unapproved methods / materials for insulating motor connections could result in the short-circuiting of motor leads, phase to phase or phase to ground, rendering the equipment inoperable, thus affecting the design function of the safety-related systems. There fore, had this condition remained uncorrected, it could have adversely affected the safety of operations of the plant.
Corrective Action The unapproved insulating materials have been removed and the terminations have been repaired with approved materials in accordance with Engineering and Design Coordination Report No. 52,527A and Nonconformance and Disposition
