Gentlemen:
SUBJECT: REPORT NOS. 50-413/85-16 AND 50-414/85-12
On April 8 - May 3,1985, NRC inspected activities authorized by NRC Operating
License No. NPF-35 and Construction Permit No. CPPR-117 for your Catawba
facility. At the conclusion of the inspection, the findings were discussed with
those members of your staff identified in the enclosed inspection report.
Areas examined during the inspection are identified in the report. Within these
areas, the inspection consisted of selective examinations of procedures and
representative records, interviews with personnel, and observation of activities
in progress.
The inspection findings indicate that certain activities violated NRC require-
ments. The violations, references to pertinent requirements, and elements to be
included in your response are presented in the enclosed Notice of Violation.
In addition to the need for corrective action regarding the specific matters
identified in the enclosed Notice, we are concerned about the implementation of
your management controls that permitted these situations to develop.
Of
particular concern is the apparent lack of caution and control demonstrated in
the decision to proceed with testing after the first overpressure incident
without thoroughly reviewing all relief valve paths and verifying that all
systems and components had adequate overpressure protection. Consequently, your
response should describe those particular actions taken or planned to improve the
effectiveness of your management control system, not only for this specific
instance, but for your overall preoperational and startup test program.
The responses directed by this letter and the enclosures are not subject to the
clearance procedures of the Office of Management and Budget issued under the
Paperwork Reduction Act of 1980, PL 96-511.
