Areas Inspected:
Routine, announced safety inspection conducted to assess the adequacy of the licensee's activities authorized under the general license provided by 10 CFR 150.20 (Recognition of Agreement State Licenses).
The inspection included a review of training on operating and emergency procedures, training on radiation safety, posting, qualification of personnel, surveys, personnel dosimetry, security, and transportation.
Results: Of the areas inspected, no apparent violations were identified.

Introduction The Amersham Corporation is authorized by the State of Illinois, an Agreement State, to install, replace and dispose-of sealed sources in dry and wet source-storage irradiators throughout the State of Illinois.
On December 18, 1992, Richard Rees and Tony Parsons, two licensed technicians, installed 39 cobalt-60 sealed sources - Amersham Model.
CKA-LSA-into a wet source-storage D radiator operated by Isomedix Operations, Inc. under NRC License No. 34-19769-04. The cobalt-60 sealed sources contained approximately 10,000 curies each, and totaled 400,000 curies of cobalt-60.
The addition of the sealed sources increased the curie strength to 2.7 million curies. Mr. Rees and Mr. Parsons provided training to two crane equipment operators who were contracted to unload the shipping cask from a flat bed transport trailer, then raise the cask to the roof of the irradiator building, and lower the cask to the bottom of the 28-foot-pool of water located inside the building. Mr. Rees and Mr. Parsons did not complete the installation until Saturday afternoon on December 19, 1992.
Later that day, personnel from Isomedix Radiation Safety Office performed a survey of the irradiator and its surroundings.

Notification Amersham Corporation notified the U.S. Nuclear Regulatory Commission on-December 3, 1992, to report proposed activities at the Isomedix Operations, Inc. facility,-Groveport, Ohio under the authority of the general license pursuant to 10 CFR-150.20.
No violations of NRC requirements were identified.
4.
Trainina Mr. Rees and Mr. Parsons submitted their qualifications to the State of Illinois to support Amendment No. 43 of Amersham's Illinois' License.
Both have had extensive training in loading, installing, removing and f

4 disposing of high activity, high energy radiation sources associated with industrial, medical and research irradiators.
Their experience covered source loading into more than 100 installations.
Isomedix Operations personnel were trained during the first installation of sources and prior to operation of the irradiator in late 1983. They have been retrained at least annually since 1983.
No violations of NRC requirements were identified.

Area Survey The irradiator and surrounding areas were surveyed after installation as required by the license.
The NRC inspector performed surveys of the shipping cask and the pool irradiator during the loading of two modules of the source rack. The surveys on that day, December 18, 1992, were in close agreement with the Amersham and Isomedix survey results.

The survey instrument (Serial Number 013164) was last calibrated on August 18, 1992.
No violations of NRC requirements were identified.

Personnel Dosimetry Each individual wore a wholebody dosimetry device and a pocket dosimeter during the loading procedure.
Pocket dosimeter results were recorded daily, and indicated no radiation exposure to any of the individuals s
involved with the loading of the Co-60. The wholebody badges were submitted for processing, and will be reviewed during the next routine inspection of the Isomedix facility.
No violations of NRC requirements were identified.

Leak Test Certificate Mr. Rees presented Isomedix valid leak test results performed prior to shipping the cask from the United Kingdom. Mr. Rees performed another leak test prior to moving the cask from the trailer. The test was performed by attaching air hoses to a drain plug at the bottom of the cask and to a similar area at the top of the cask.
Air, under low pressure, was circulated through the bottom of the cask through a filter attached to the air hose exiting the top of the cask. After approximately five minutes, the filter was removed from the upper air line and counted in a well counter coupled with a Ludlum Model 2200 Scaler ratemeter single channel analyzer. No radioactive contamination was detected on the filter.
No violations of NRC requirements were identified.

Security

Only trained Isomedix personnel will have keys to the room where the irradiator is located.

9.
Transoortation The shipping cask was transported from England by air via Federal Air Express to Memphis, Tennessee, as a "Special Arrangement" transport movement. It was transported from Memphis, Tennessee to Groveport, Ohio on a special equipped tractor - trailer vehicle.
It was transported
" Highway route controlled quantity" as described in 49 CFR 173.403(1)(h).
It was properly marked, labelled and the truck placarded as required.
The tractor-trailer arrived at the Isomedix facility on December 15, 1992. The cask was surveyed and leak tested upon arrival.
No contamination was detected from the wipe tests.
The radiation readings were similar to those listed under Section 5.
The trailer was secured and posted until December 18, 1992, when the cask was removed.
No violations of NRC requirements were identified.

Exit Meeting On December 18, 1992, an exit meeting was held with those individuals denoted in Section 1.
The meeting included a discussion of the inspection findings.
None of the information in this report was identified as proprietary by the licensee.

Even though the new rule does not become effective until January 1, 1993, we have implemented procedures to accommodate this now rule.
