Arkansas Nuclear One, Unit 1 e UNPLANNED RELEASE NUMBER 1 Description On February 6, 1992, during testing of t he "A" Waste Gas Storage Tank (T-18A) under a job order, an unplanned release occurred.
Cause:
The purpose of the job order was to twplace all isolation valves associated with the Waste Gas System (T-18). During this work evolution, testir.3 of the valves were requirr.d by pressurization of the system with nitrogen. Upon completion of the testing, T-18A was at 50 psi.
It was then noticed that approximately 15 psi was lost from T-18A and T-18D had gained approximately 15 psi. This change in pressure was caused by a leaking valve that had not been replaced. To continue the testing evolution, both tanks, T-18A and D had to ie depressurized.
A release permit was prepared to release both tanks. B?th tanks were sampled for radioactivity. Upon release of T-18D, T-18A was also released due to the leaking valvo. 1.159E-3 curies and 22.8 cubic meters of effluent was released from T-18A.
Actions:
Interim actions taken to ensure no other unplanned releases occurred during the< testing was that communications between the departments involved with the job order was increased.
Permanent actior s consisted of the Waste Gas System being upgraded with soft seat vaivta. This upgrade should climinate the leakage of gas through these valves.
Conseq'rences :
Because T-18A was pressurized with nitrogen for testing, the radioactive contents of this tank was considerably lower than normal tank activities.
Upon release, the tank had a total radioactive content of 1.159E-3 curies. This release produced a quarterly critical organ-dose of 3.852E-4 mrem. This represents 0.005% of the quarterly limits.
The activities released from this tank were low as compared to nr.rmal routina T-18 releraes. No abnormal consequences should occut fu m this release.

During the quarterly performance of the Process Radiation. System Test, the ANO-.1 liquid (RE/RI-4642) effluent monitor failed. A job order was initiated to_ repair the component. Based on station logs, the monitor.
was out of service from March 17, 1992, until April 22, 1992.
The job order referenced the Technical Specification, but did not
. identify that a Limiting Condition of Operations'(LCO) and a time clock ;
were involved. - ANO-1 was in a refueling outage (IR10) at that time.

Since -no I.C0 or time limit was identir:cd in the job order, the job order did not receive a suf ficient priority to complete the- job unit April 22, 1992. A Condition Report was issued to address this overright.

I ATTACIIMENT D CllANGES TO TiiE OFFSITE DOSE CALCULAT10NAL MANUAL AND PROCESS CONTROL PROGRAM

I On January 16, 1992, the ANO Safety Review Committee (SRC) reviewed and approved Revision 2 to the Offsite Dose Calculation Manual (ODCM). This revision became effective upon that date.
In accordance with ANO-1 Technical Specification 6.14.2 and ANO-2 Technical Specification 6.14.2, a copy of Revision 2 to the ODCM can be found in Attachment E.
The main objectives of this revision are to:
- 1) update the status of the environmental monito.ing sample locations;
- 2) update the status of the monitors used by the radiological effluent release software;
- 3) remove the tables from the body of the ODCM.
Previded below is a detailed discussion of the changes to the ODCM.
- Section 2.1 now specifically delineates the liquid radwaste monitors covered by this methodology.
- The terms " SLOPE" and "0FFSET" in Section 2.1.5 have been redefined so they no longer contain the words " slope" and " offset" in their definitions.
- The note in Section 2.2.1 regarding potable water was removed since Russellville City Water may not only come from the Illinois Bayou during times ot water shortages and Secticn 2.2.2 was added to describe the calculations.to be used during those times.
- The discussion of tne Bioaccumulation factor was simpli~ied and refers to AN0's letter to NRR dated April 13, 1984 (0CAN048408), for a more detailed description of the factor.
- ' The monitors RE-7400, 2RE-8540, 2RF>8542, and 2RE-7828 have been deleted from Section a.1.1.a and its overall discussion simplified.
Since the use of these monitors were Jn conjunction with Appendix B effluent database software and not the RETS effluent database software.
e The setpoint methodology has been clarified and now includes a factor of 2.0 times background to allow for random fluctuations in readings.
- The Low Level Radwaste Building SPING (2RX-9850) has been added to Section 3.1.2.a.
- The use of the word poultry in Section 3.4 has been deleted since maximum individual ?ose methodologies from Regulatory Guide 1.109 and NUREG-0133 for ineat only use cattle.

- Section 3.6 has been added to describe the dose calculation to the public inside the site bocndary,
- The environmental monitoring sample locations have been updated in Table 4-1 and Figure 4-1 to reflect the current locations in use from the results of the 1991 Land End Use Census.
These changes do not degrade the effectiveness of the ODCM or contradict any commitments made to the NRC regarding its content or use. These changes-will not reduce the accuracy or reliability of dose calculations or setpoint determinations.
This revision was also a complete reformat of the ODCM; therefore the entire ODCM is being submit ted.
No changes to the Process Control Program were made during the first and second quarters of 1992, i
