SUBJECT:

SHNPP Unit No.1 - 10CFR50.55 (e) and 10CFR21, reportable deficiency. Air start valve capscrews for emergency diesel generators purchased from Transamerica DeLaval, Inc. under P.O. # NY-435079 ITEM: Air start valve capscrews in. stalled in diesel engine cylinder head assemblies. Total of 64 capscrews in two diesel engines.

SUPPLIED Bl: Transamerica DeLaval, Inc. (TDI), Oakland, California NATURE OF DEFICIENCY: TDI shipped two diesel engines to the CP&L site on P. O. #

NY-435079 in May and June 1981.

In June 1982, TDI advised CP&L that the tapped holes in the cylinder heads may not be tapped deep enough to accommodate the air start valve capscrews, causing the capscrews to bottom out and lose preload. The old part from TDI was a 3-inch long capscrew, the new part is 2-3/4 inches long.

DATE PROBLEM OCCURRED: CP&L was notified by letter dated June 8,1982 from R. A.

Pratt (TDI) to R. M. Parsons (CP&L).

DATE PROBLEM REPORTED: February 19,1985 - CP&L (N. 3. Chiangi) notified the NRC Region II (A. Hardin) that this item was reportable under 10CFR50.55 (e) and 10CFR, Part 21.

SCOPE OF PROBLEM: The potential problem involved the two diesel generators shipped on P. O. # NY-435079. These engines had the old style 3" capscrews.

SAFETY IMPLICATION: The diesel generator units supply power to the ESF busses in case of a loss of both the normal on-site and off-site power sources.

If the capscrews bottomed out in the cylinder heads, they could fail in cyclic fatigue, thereby leading to air start system failure and subsequently, engine unavailability.

IS REPORTABLE: If left uncorrected, failure of the air start valve capscrews .

could result in loss of the emergency on-site AC power supply. j CORRECTIVE ACTION: The original 3 inch capscrews have besn replaced with new TDI part no. GB-032-Il3 which are 2-3/4 inches long. The cylinder head tapped holes and the air start valve capscrews were also measu.ed as a part of the TDI D/G Owner's Group inspection plan to assure that bottoming out would not occur.
