NIAGARA M0 HAWK POWER CORPORATION NINE MILE POINT - UNIT 2 DOCKET NO. 50-410 Final Report for a Problem Concerning the Incorrect Switches Installed on PGCC Panel (55(e)-8E-06)
Description of the Problem The problem concerns the installation of spring return three-position switches in lieu of the specified two-position switches on panel 2 CEC *PNL601.
The panel was suppl ied by General El ectric.
These switches are designed for manual operation of low pressure core spray, high pressure core spray, and residual heat removal water leg pumps.
The spring-return switches would not pennit the continuous operation of these pumps, as required by system design, unless the operator holds the switch in the "run" position.
Analysis of Safety Implications If the lines mentioned above are not filled prior to operation, system integrity could be jeopardized due to water hammer loads.
Therefore, if this condition were to have remained uncorrected, it could have adversely affected the safety of operation of the plant.
Corrective Action The subject switches have been replaced with the correct switches.
