This refers to a 10 CFR 50.72 reportable event which occurred at 1510 CDT on July 4, 1984. Control room ventilation and containment purge isolation signals were initiated due to a loss of power to three process radiation monitors.
Further investigation by your staff revealed manually opened, rather than tripped, power supply breakers in the 120V distribution panel which supplies the radiation monitors.
Based on discussions between you and Mr. C. E. Norelius of my staff, it is our understanding that prior to the Plant Manager's approval to take the reactor critical, you will:
(1) conduct a thorough investigation of this event to deter-mine the most probable causes; and (2) provide for NRC review and acceptance, the results of your investigation and corrective actions taken to prevent recurrence.
Please let us know immediately if your understanding differs from that set forth above.

ACTS OF VANDALISM OR SABOTAGE Receipt of an operating license places a responsibility on each of us that must be fully understood.
Any malicious act of vandalism or sabotage at a nuclear power plant that could jeopar-dize the safety of the general public, plant personnel,or plant safety is a violation of Federal and State laws and will be dealt with in a severe manner by Union Electric Company.
You should be aware that penalties under the law may be extended to individuals who are passively involved (have information but fail to report it) as well as to those who are actively involved in acts of van-dalism or sabotage.
Unauthorized alteration of valve or breaker alignment is so categorized, irrespective of the actual consequences of the alteration.
Such violations can involve penalties of up to S10,000 and 10 years imprisonment.
I would hope that acts of this kind will not occur at Callaway.
However, be advised that Union Electric Company will exercise the necessary resources including the use of plant security forces, as well as local, State and Federal law enforcement agencies in dealing with such acts.
We call upon each of you to' assist us in identifi-cation of any person engaged in unauthorized or suspisious activities which might be considered vandalism or sabotage.
If you witness such an act or have information relevant to known or suspected acts of vandalism or sabotage, you are obliged to repor it to your manage-ment.
